信息-知识-信念-行为模式健康教育在口服磷脂酰肌醇3激酶抑制剂的淋巴瘤患者血糖管理中的应用

摘要
目的 探讨信息-知识-信念-行为(IKAP)模式的健康教育在淋巴瘤患者磷脂酰肌醇3激酶(PI3K)抑制剂服药期间高血糖管理中的应用.方法 选取2018年11月至2023年10月在天津市人民医院收治的96例接受PI3K抑制剂单药治疗淋巴瘤患者作为研究对象,随机分为对照组及研究组.其中对照组(48例)在用药期间实施常规健康教育,研究组(48例)在常规健康教育基础上予IKAP模式的健康教育.监测两组患者第1、2、3、7、10、14天的空腹、早餐后2h、午餐后2h的指血血糖.结果 研究组实施IKAP模式健康教育护理后,患者控制饮食、适当运动和监测血糖的积极性和主动性明显提高,两组比较差异均有统计学意义(均P＜0.05).干预后,研究组经过IKAP模式的健康教育及高血糖分层管理,第2、3、7、10、14天空腹、早餐后2h、午餐后2 h血糖与对照组比较,差异均有统计学意义(均P＜0.05).结论 对服用PI3K抑制剂的恶性肿瘤患者给予规律血糖监测及IKAP模式健康教育是非常有必要的,能够显著改善患者血糖情况,降低其高血糖的发生率,为抗肿瘤治疗提供帮助.
Abstract
Objective To evaluate the application of health education based on the information-knowledge-attitude-practice(IKAP)model in lymphoma patients with hyperglycemia undergoing PI3K inhibitor treatment.Methods Ninety-six lymphoma patients receiving PI3K inhibitor monotherapy for malignant tumor treatment admitted to Tianjin Union Medical center from November 2018 to October 2023 were randomized into a control group receiving routine health education(48 cases),and a study group receiving IKAP mode of health education(48 cases).Levels of patients'blood glucose were monitored at different time points,including fasting glucose,blood glucose 2 hours after breakfast,and blood glucose 2 hours after lunch on days 1,2,3,7,10 and 14.Results After implementation of the IKAP model of health education and nursing,the patient's enthusiasm and initiative in controlling diet,exercising appropriately,and monitoring blood sugar were significantly improved,and the differences between the two groups were statistically significant(all P＜0.05).After IKAP mode of health education and hierarchical management of hyperglycemia,significant differences in fasting glucose,blood glucose 2 h after breakfast and blood glucose 2 h after lunch on days 2,3,7,10 and 14 were observed between the study group and the control group(all P＜0.05).Conclusion Regular blood glucose monitoring with IKAP model of health education in cancer patients undergoing PI3K inhibitor treatment is necessary,which could significantly lower patients'blood glucose level and reduce the incidence of hyperglycaemia during anti-tumor therapy.
