补肾通络汤联合仙灵骨葆胶囊治疗老年骨质疏松性桡骨远端骨折临床研究

摘要

目的:探讨补肾通络汤联合仙灵骨葆胶囊治疗骨质疏松性桡骨远端骨折的临床疗效.方法:选取我院2021年1月—2022年12月收治的骨质疏松性桡骨远端骨折患者124例,按随机数字表法分为观察组与对照组各62例,对照组患者给予西医常规治疗,观察组在对照组基础上给予补肾通络汤联合仙灵骨葆胶囊治疗,比较两组治疗前后临床疗效,中医证候评分,以及骨折愈合时间、肿胀消退时间和疼痛缓解时间,血清骨代谢指标(血清骨钙素、Ⅰ型前胶原氨基端前肽、Ⅰ型胶原C端肽β降解产物),骨密度水平,腕关节功能及并发症情况.结果:治疗后观察组总有效率(94.92％)显著高于对照组(84.75％),差异有统计学意义(P＜0.05);治疗后两组中医证候评分、血清Ⅰ型胶原C端肽β降解产物水平均较治疗前降低,且观察组低于对照组(P＜0.05);治疗后观察组骨折愈合时间、肿胀消退时间及疼痛缓解时间均短于对照组(P＜0.05);治疗后两组血清骨钙素、Ⅰ型前胶原氨基端前肽及骨密度水平、掌倾角、尺偏角角度均较治疗前升高,且观察组高于对照组(P＜0.05);治疗后观察组并发症发生率(6.78％)显著低于对照组(20.34％),差异有统计学意义(P＜0.05).结论:补肾通络汤联合仙灵骨葆胶囊治疗老年骨质疏松性桡骨远端骨折临床疗效显著.

Abstract

Objective To investigate the clinical effect of Bushen Tongluo decoction combined with Xianling Gubao capsule on osteoporotic distal radius fracture.Methods A total of 124 patients with osteoporotic distal radius fracture were selected from our hospital and divided into the observation group and the control group with 62 patients in each group according to the method of random number table.Control group was given conventional western medicine treatment,observation group was given Bushen Tongluo decoction combined with Xianling Gubao capsule on the basis of control group.To observe the clinical efficacy,TCM syndrome scores,fracture healing time,swelling subside time and pain relief time of the two groups were observed before and after treatment.Serum bone metabolic indices(serum osteocalcin,amino terminal propeptide of type Ⅰprocollagen,C-terminal peptide β degradation products of type Ⅰ collagen),bone density level,wrist joint function and complications.Results After treatment,the observation group total effective rate was 94.92％significantly higher than that of control group 84.75％(P＜0.05).TCM syndrome score and serum level of C-terminal peptide β degradation product of type Ⅰ collagen in 2 groups were lower than before treatment,and observation group was lower than control group(P＜0.05).After treatment,fracture healing time,swelling regression time and pain relief time in observation group were shorter than those in control group(P＜0.05).After treatment,serum osteocalcin,amino terminal propeptide of serum type Ⅰ procollagen,bone mineral density,palm inclination angle and ulnar declination angle in 2 groups were increased compared with before treatment,and observation group was higher than control group(P＜0.05).After treatment,the complication rate of observation group was 6.78％,which was significantly lower than that of control group(20.34％)(P＜0.05).Conclusion Bushen Tongluo decoction combined with Xianling Gubao capsule has significant clinical effect on senile osteoporotic distal radius fracture.
