阴茎交感神经皮肤反应评估养阴清热中药治疗阴虚火旺型早泄患者疗效的可行性研究

摘要
目的:探讨养阴清热中药(加减知柏地黄汤)治疗阴虚火旺型早泄患者的临床疗效及阴茎交感神经皮肤反应(PSSR)评估在其中的应用价值.方法:选取2022年1月-2023年12月浙江大学医学院附属第二医院收治的阴虚火旺型原发性早泄(PPE)患者共96例,随机将其分为治疗组(养阴清热中药)与对照组(盐酸舍曲林片50 mg)两组,治疗周期均为8周.观察两组患者治疗前后阴道内潜伏时间(IELT)、早泄诊断量表(PEDT)、PSSR潜伏期和波幅、国际勃起功能指数-5(IIEF-5)的变化.结果:治疗前和治疗后,两组IELT、PEDT、PSSR波幅、PSSR潜伏期比较,差异无统计学意义(P＞0.05);治疗后,两组IELT、PSSR潜伏期均高于治疗前,PEDT、PSSR波幅低于治疗前,差异有统计学意义(P＜0.05).两组治疗前后IIEF-5相比,差异均无统计学意义(P＞0.05).结论:养阴清热中药治疗阴虚火旺型PPE的疗效肯定,PSSR潜伏期是其临床疗效的一种较为客观的评价指标.
Abstract
Objective Exploring the clinical efficacy of traditional Chinese medicine for nourishing Yin and clearing heat(modified Zhibai Dihuang decoction)in the treatment of premature ejaculation patients with Yin deficiency and hyperactive fire,and the application value of penile sympathetic skin response(PSSR)evaluation in this regard.Methods 96 patients with primary premature ejaculation(PPE)of Yin deficiency and hyperactive fire type,in outpatient clinics were selected from January 2022 to Decembe 2023,randomly divided into two groups:the treatment group(traditional Chinese medicine for nourishing yin and clearing heat)and the control group(50 mg of sertraline hydrochloride tablets),with a treatment period of 8 weeks for both groups.Compare the intravaginal latency time(IELT),premature ejaculation diagnostic scale(PEDT),International Erectile Function Index-5(IIEF-5),and PSSR latency and amplitude detected by electromyography evoked potential before and after treatment.Results Before and after treatment,there was no statistically significant difference in IELT,PEDT,PSSR amplitude,and PSSR latency in the two groups(P＞0.05);After treatment,the incubation periods of IELT and PSSR in both groups were higher than before treatment,while the amplitudes of PEDT and PSSR were lower than before treatment,the difference is statistically significant(P＜0.05).There was no statistically significant difference in IIEF-5 between the two groups before and after treatment(P＞0.05).Conclusion Positive therapeutic effect of Traditional Chinese Medicine of nourishing yin and clearing heat in the treatment of primary premature ejaculation.The latent period of PSSR is a relatively objective evaluation index for evaluating the clinical efficacy of Traditional Chinese Medicine in the treatment of primary premature ejaculation.
