基于"线粒体-氧化应激"损伤机制从"痰"论治血管性痴呆

血管性痴呆(vascular dementia,VaD)多继发于脑血管病,其患病率高,发病机制不明.脑血管病使神经元缺血、缺氧,进而诱发线粒体形态和功能异常,大量强氧化物质释放,最终造成神经元氧化损伤,因此"线粒体-氧化应激"损伤机制是VaD认知障碍的关键机制.本文通过梳理和总结国内外"线粒体-氧化应激"损伤机制的研究和报道,将氧化应激损伤和中医学呆病关键病理因素"痰浊"结合,从而建立VaD"痰浊-氧化应激-神经炎症"的中西医结合病理理论,在此基础上深入探讨VaD认知障碍修复机制和潜在治疗靶点,论述"治痰法"的有效性和科学性,为中医药防治VaD提供新的思路.
Treatment of Vascular Dementia from the Perspective of"Phlegm"Based on the Damage Mechanism of"Mitochondrial-Oxidative Stress"
Vascular dementia(VaD)is often secondary to cerebrovascular disease,with a high incidence and unclear pathogenesis.Cerebrovascular disease causes neuronal ischemia and hypoxia,leading to abnormal mitochondrial morphology and function.A large amount of strong oxidizing substances are released,ultimately causing neuronal oxidative damage.Therefore,the"mitochondrial oxidative stress"damage mechanism is the key mechanism of VaD cognitive impairment.This article summarized the research and reports on the mechanism of mitochondrial oxidative stress damage both domestically and internationally,and combined oxidative stress damage with the key pathological factor of traditional Chinese medicine about dementia,"phlegm",to establish a pathological theory of"phlegm-oxidative stress-neuroinflammation"of VaD,combining traditional Chinese medicine and western medicine.Based on this,we could study the repair mechanism and potential therapeutic targets of VaD cognitive impairment,and discuss the effectiveness and scientificity of"phlegm treatment method",to provide new ideas for the prevention and treatment of VaD with traditional Chinese medicine.
