目的 探索芪术肺纤汤治疗特发性肺间质纤维化(idiopathic pulmonary fibrosis,IPF)患者的临床疗效.方法 纳入 40例2019 年 10 月1 日至 2021 年12 月 31 日于中国中医科学院广安门医院就诊及住院的IPF患者,随机分为试验组和对照组各 20例,对照组患者口服乙酰半胱氨酸胶囊加百令胶囊,试验组患者在此基础上加用芪术肺纤汤,治疗 3 个月后观察 2 组患者肺功能、6 分钟步行距离(six-minute walk distance,6MWD)、静息状态血氧饱和度(oxygen saturation,SpO2)、圣乔治呼吸问卷(the Saint George's respiratory questionnaire,SGRQ)评分、血清涎液化糖链抗原6(krebs von den lungen-6,KL-6)水平、高分辨CT(high-resolution ct,HRCT)半定量评分变化.结果 治疗后试验组患者一氧化碳弥散量占预计值的百分比(carbon monoxide diffusing capacity/predict value,DLCO%)明显高于对照组,差异有统计学意义(P<0.01);治疗后试验组SGRQ总分、症状评分、疾病影响评分、血清KL-6和HRCT半定量评分均低于对照组,差异有统计学意义(P<0.05);肺功能分级和HRCT半定量评分呈正相关性(r=0.849,P<0.01).结论 芪术肺纤汤能够延缓IPF患者的肺纤维化进展,值得临床推广应用.
Clinical Observation on the Therapeutic Effect of Qizhu Feixian Decoction on Idiopathic Pulmonary Interstitial Fibrosis
Objective To explore the clinical effect of Qizhu Feixian decoction on idiopathic pulmonary fibrosis(IPF)patients.Methods A total of 40 IPF patients admitted to Guang'anmen Hospital,China Academy of Chinese Medical Sciences from October 1st,2019 to December 31st,2021 were included,and divided into trial group and control group at random,with 20 cases in each group.The control group was given acetylcysteine capsules plus Bailing capsules,and the trial group was given Qizhu Feixian decoction in addition.After 3 months of treatment,the pulmonary function,6-minute walk distance(6MWD),resting oxygen saturation(SpO2),the sores of the Saint George's respiratory questionnaire(SGRQ),serum krebs von den lungen(KL)-6 level,and the semi-quantitative scores of high-resolution ct(HRCT)were observed in both groups.Results After treatment,the carbon monoxide diffusing capacity/predict value(DLCO%)in the trial group was obviously higher than that in the control group(P<0.01).After treatment,the total scores of SGRQ,the scores of symptom,the scores of influence of disease,the level of KL-6,and the scores of HRCT in the trial group were lower than those in the control group(P<0.05).There was a positive correlation between lung function grade and HRCT semi-quantitative scores(r=0.849,P<0.01).Conclusion Qizhu Feixian decoction can delay the progression of pulmonary fibrosis in patients with IPF.It is worthy of further promotion.
