基于SrTO法探讨当代中医名家论治出血性中风的学术特点

目的 探究当代中医医家论治出血性中风的学术特点,为临床诊治提供思路与方法.方法 全面系统检索当代中医名家论治出血性中风相关文献.依据澳大利亚循证方法学组织(Joanna Briggs Institute,JBI)研发的基于文本和专家意见的系统综述(the systematic review of text and opinion,SrTO)方法,对中医脑病领域名医名家诊治出血性中风临证经验、学术思想等相关研究进行证据整合,并从理法方药、因机证治多方面分析其学术特点及影响因素.结果 共纳入 36 名中医医家,医家在华南、华东、华北、华中等医疗发达地区分布多,其中北京地区纳入医家数量最多,为7 名;纳入医家的20%有中医家传教育背景,69.4%有师承名医学习经历;36 名医家总结归纳形成包括脑髓理论、伏邪理论、毒损脑络在内共计 11 种新理论;创新性地提出包括通腑化痰、破血化瘀、痰瘀同治在内共计23 种新治法.结论 当代中医名家将传统理论和现代研究相融合,诊治上经典理论与临床实践并重,学术上传承与创新兼备,对出血性中风的研究逐渐深入.
Exploring the Academic Characteristics of Contemporary Famous TCM Doctors in Treating Hemorrhagic Stroke Based on the SrTO Method
Objective To explore the academic characteristics of contemporary traditional Chinese medicine(TCM)doctors in treating hemorrhagic stroke,and to provide ideas and methods for clinical diagnosis and treatment.Methods A comprehensive and systematic search was made for the academic literature on the treatment of hemorrhagic stroke by famous TCM practitioners.Based on the systematic review of text and opinion(SrTO)methodology developed by the Joanna Briggs Institute(JBI)in Australia,the clinical experience and academic ideas of the famous doctors in the field of TCM encephalopathy in the treatment of hemorrhagic stroke were integrated.And the academic characteristics and influencing factors were analyzed from the aspects of theory,therapy,prescription and medicine.Results A total of 36 TCM doctors were included.And the doctors were distributed in developed areas such as South China,East China and North China.Beijing area included 7 doctors,which was the largest number among them.20%of them had family heritage education background.69.4%of them had studying experience from famous doctor.They innovatively put forward a total of 11 new theories,including phlegm-blood stasis theory,brain marrow theory and latent pathogens theory.23 new methods were presented,including purging Fu-organs and resolving phlegm,breaking blood and removing blood stasis,and the treatment of phlegm and stasis together.Conclusion Contemporary Chinese medicine experts integrate traditional theory with modern research,placing equal emphasis on classic theory and clinical practice in diagnosis and treatment,and combining academic inheritance and innovation,gradually deepening their research on hemorrhagic stroke.
