Research on Diagnostic Criteria for Liver Stagnation and Spleen Deficiency Syndrome in Hyperlipidemia Based on Delphi Method and Analytic Hierarchy Process
Objective Based on the delphi method and analytic hierarchy process,evaluate the weight of clinical symptoms and tongue pulse of hyperlipidemia with liver stagnation and spleen deficiency syndrome,in order to explore the diagnostic criteria for this syndrome. Methods 15 clinical experts from top tier hospitals across the country who have been engaged in traditional Chinese medicine and integrated traditional Chinese and Western medicine clinical work for more than 10 years were selected,and brainstorming was used to list common clinical symptoms,tongue manifestations,and pulse conditions of hyperlipidemia and liver stagnation and spleen deficiency syndrome. The symptoms were combined to determine the item pool for the diagnostic scale. Delphi method was used for expert consultation to ultimately determine the item indicators,combined with the analytic hierarchy process to calculate the proportion as the item weight,in order to explore the contribution of various diagnostic elements of hyperlipidemia with liver stagnation and spleen deficiency syndrome. Results The item pool for hyperlipidemia with liver stagnation and spleen deficiency syndrome was determined by brainstorming,with 17 clinical symptoms,11 tongue manifestations,and 10 pulse conditions. 30 valid questionnaires were received by the delphi method,with a total expert positive coefficient of 100%. The analytic hierarchy process resulted in the ranking of primary indicators in descending order of clinical symptoms,tongue manifestations,and pulse conditions. The secondary indicator weights ranked from highest to lowest were as below. Clinical symptoms:Fatigue,irregular bowel movements,confusion,irritability,bloating,etc. Tongue manifestations:Moss white and greasy,plump tongue,dented tongue,etc. Pulse conditions:string pulse,slippery and string pwlse,slippery pulse,etc. Conclusion Based on brainstorming,delphi method,and analytic hierarchy process,the importance of diagnostic criteria for hyperlipidemia with liver stagnation and spleen deficiency syndrome can be effectively evaluated,which is a feasible exploration for the study of diagnostic criteria for this syndrome.
