Effect of Butylphthalide Combined with Extract of Ginkgo Biloba Leaves on Hemodynamics and Serum Neurotrophic Factors in Patients with Acute Ischemic Stroke
Objective:To investigate the effect of butylphthalide combined with extract of ginkgo biloba leaves on hemodynamics and serum neurotrophic factors in patients with acute ischemic stroke(AIS).Methods:A total of 147 patients with AIS admitted to a hospital from January 2020 to April 2022 were randomly divided into the ginkgo biloba leave group,the butylphthalide group and the combination group according to random number table,with 49 patients in each group.The patients in the three groups were all treated with routine therapy including aspirin enteric-coated tablets,the ginkgo biloba leave group was treated with extract of ginkgo biloba leaves injection in addition to the routine therapy,the butylphthalide group was treated with butylphthalide and sodium chloride injection in addition to the routine therapy,and the combination group was treated with butylphthalide and sodium chloride injection in addition to the treatment given in the ginkgo biloba leave group.The National Institute of Health stroke scale(NIHSS),Barthel index(BI),stroke-specific quality of life scale(SS-QOL)scores,hemodynamic parameters(mean blood flow velocity,dynamic resistance,peripheral resistance),serum levels of neurotrophic factors[glial cell line-derived neurotrophic factor(GDNF),brain-derived neurotrophic factor(BDNF)],serum glial fibrillary acidic protein(GFAP),homocysteine(Hcy),hypersensitive C reactive protein(hs-CRP),clinical response and occurrence of adverse reactions were compared among the three groups.Results:After treatment,the NIHSS scores were decreased in all of the three groups(P<0.05),and were lower in the combination group than those in the ginkgo biloba leave group and butylphthalide group(P<0.05).The BI and SS-QOL scores were increased in all groups(P<0.05),and were higher in the combination group than those in the ginkgo biloba leave group and butylphthalide group(P<0.05).The peripheral resistance and dynamic resistance were decreased in the three groups(P<0.05),and were lower in the combination group than those in the ginkgo biloba leave group and butylphthalide group(P<0.05).The mean blood flow velocity were increased in the three groups(P<0.05),and were higher in the combination group than that in the ginkgo biloba leave group and butylphthalide group(P<0.05).The serum levels of GDNF and BDNF were increased in the three groups(P<0.05),and were higher in the combination group than those in the ginkgo biloba leave group and butylphthalide group(P<0.05).The serum levels of GFAP,Hcy and hs-CRP were decreased in the three groups(P<0.05),and were lower in the combination group than those in the ginkgo biloba leave group and butylphthalide group(P<0.05).The total response rate was higher in the combination group than that in the ginkgo biloba leave group and butylphthalide group(x2=6.019,P=0.049).There was no significant difference in the total incidence of adverse reactions among the three groups(x2=0.632,P=0.729).Conclusion:Butylphthalide combined with extract of ginkgo biloba leaves is effective in restoring the neurological function and improving the quality of life,cerebral hemodynamics and serum levels of neurotrophic factors in patients with AIS,demonstrating definite clinical efficacy and medication safety.
