Effect of Clotrimazole on Tacrolimus Trough Concentration after Renal Transplantation
Objective:To investigate the effect of clotrimazole tablets on tacrolimus trough concentration after renal transplantation.Methods:A total of 120 patients who underwent renal transplantation in the Department of Renal Transplantation of a hospital from January to December 2020 were included in a retrospective study,and assgined into clotrimazole group(n=56)or non-clotrimazole group(n=64)according to whether clotrimazole was used together.The study compared the time taken to reach the tacrolimus trough concentration target,allograft biopsy rate,incidence of acute cell rejection and oropharyngeal candida infection tablets,as well as the changes in tacrolimus trough concentration before and after clotrimazole tablets between the two groups.Results:After discontinuation of clotrimazole tablets,the median tacrolimus trough concentration decreased from 10.60ng/ml(8.48ng/ml,12.33ng/ml)to 6.75ng/ml(5.50ng/ml,8.68ng/ml)(P<0.0001).Patients receiving clotrimazole tablets after renal transplantation achieved the first tacrolimus trough concentration in a shorter time than those without clotrimazole(P=0.0023).No significant difference in the incidence of allograft biopsy and oropharyngeal candida infection between the two groups(P>0.05).No clear acute cell rejection was observed in the clotrimazole group,while 3 patients in the non-clotrimazole group experienced acute cell rejection.Conclusion:Drug interaction between tacrolimus and clotrimazole was observed after renal transplantation,and the withdrawal of clotrimazole resulted in a significant decrease in the median trough concentration of tacrolimus.
