Analysis on the Rationality of Clinical Use of Warfarin in a Hospital in 2020
Objective:To analyze the use of warfarin in a certain hospital in 2020,and to explore the rationality of relevant clinical use.Methods:The patients who were treated with warfarin from January to December in 2020 were selected using Donghua Standard Edition Digital Hospital Information Management System in a certain hospital,and their sex,age,department distribution,clinical diagnosis,route of administration and dosage,international normalized ratio(INR),combination medication and adverse drug reactions were analyzed retrospectively.Results:A total of 324 patients were collected,including 178 males and 146 females,with a male-to-female ratio of 1.22:1.The patients were mainly aged of 70 to 79 years old(87 cases,26.85%).They were mainly distributed in the cardiovascular department(140 cases,43.21%),and the main clinical diagnoses were atrial fibrillation(163 cases,50.31%)and relevant diagnoses after cardiac valve replacement(51 cases,15.74%).The INR was 2 or higher but no more than 3.5 in 112 patients(34.57%),and normal in 89 patients(27.47%).There were a total of 109 chemical drug combination medications,including cardiovascular system drugs(such as atorvastatin),anti-infective drugs(such as levofloxacin),digestive system drugs(such as omeprazole),and endocrine drugs(such as metformin).The adverse reaction records were available in only 15 hospitalized patients,with the main relevant symptoms being gastrointestinal haemorrhage and subcutaneous ecchymosis.Conclusion:The clinical application of warfarin in this hospital is not standardized,and both the INR test rate and the normal rate are low.
