Characteristics Analysis of Bacillus Cereus Endophthalmitis and Drug Treatment Strategy
Objective:To analyze the characteristics and resistance of Bacillus cereus endophthalmitis and explore the drug therapy.Methods:The clinical data of 7 patients with Bacillus cereus endophthalmitis confirmed by bacteriological culture from January 2013 to December 2023 in a hospital were collected to statistically analyze the occupation,etiology,changes in visual acuity before and after treatment,surgical methods,drug susceptible test results,drug treatment strategies and economic indexes of hospitalization.Results:After surgery and drug treatment,all 7 patients with Bacillus cereus endophthalmitis retained their eyeballs at the last discharge,2 patients(28.57%)did not cooperate with visual acuity examination,and the visual acuity was decreased in 1 patient(14.29%),did not change in 1 patient(14.29%),and was increased in 2 patients(28.57%)by 1 grade and in 1 patient(14.29%)by 4 grades.The results of drug susceptible test showed that the bacterium was sensitive to vancomycin,chloramphenicol,amikacin,rifampicin,imipenem and cilastatin,but resistant to ampicillin,penicillin,cefazolin and ceftazidime.The number of hospitalization was more than 1 in 4 patients(57.14%),the number of surgeries was more than 1 in 5 patients(71.43%),the total cost of hospitalization was more than RMB 20,000 in 6 patients(85.71%),and the total days of hospitalization was more than 1 week in 7 patients.Conclusion:Bacillus cereus endophthalmitis is rare,but due to the ability to rapidly multiplicate and high migration rate of the organism,eyeball infection can induce severe inflammatory reaction and retinal structure destruction.Hence,rapid and appropriate treatment should be given,intravitreal injection of vancomycin is the primary therapy for Bacillus cereus endophthalmitis.
