摘要
阵发性睡眠性血红蛋白尿(PNH)是一种后天获得性造血干细胞基因突变导致红细胞膜缺陷的疾病,临床以血管内溶血发作,血红蛋白尿,以及全血细胞减少等为主要表现.李宏良教授认为,该病以正虚(脾肾亏虚)为本,湿、毒、瘀为标,扶正祛邪为PNH的治疗总则,补益脾肾、扶助正气为根本,辅以清热解毒、祛湿、化瘀通络,临证时应根据疾病所处的阶段而有所侧重.从脾肾论治PNH,取得较好疗效.
Abstract
Paroxymorial nocturnal hemoglobinuria(PNH)is a disease with acquired hematopoietic stem cell gene mutation resulting in the defect of red cell membrane.Clinical manifestations include intravascular hemolysis attack,hemoglobinuria,and pancytopenia.Professor Li Hongliang believes that this disease is based on positive deficiency(deficiency of spleen and kidney),with dampness,toxin and stasis as the symptoms.Strengthening the spleen and kidney,dispelling pathogenic factors should be the general treatment principle for PNH.Tonifying spleen and kidney,strengthening the healthy qi should be the root,supplemented by clearing heat and detoxifying,dispelling dampness,removing blood stasis and unblocking collaterals.During clinical practice,emphasis should be placed on the stage of the disease.Treating PNH from the perspective of spleen and kidney has achieved good efficacy.
