新安医家孙一奎与龚廷贤治疗臌胀学术思想异同探析

摘要
孙一奎和龚廷贤治疗臌胀均注重后天脾土、怒气伤肝及年老体虚患者.脾病及肝,肝病累脾,肝脾是臌胀的关键病位,治疗上,温脾提气是关键.龚廷贤认为,脾土之阴受伤是重中之重,故立足脾胃,多用健脾顺水宽中之法,方以四君子汤和平胃散为基础,同时注重药物剂型的使用,汤丸散丹,法象炮制用药,具有简便廉验之特点.孙一奎认为,水不利为下焦虚寒,因此创立壮原汤,温补下元,使火气盛而湿气蒸发,胃中温暖,谷食易化,则满可宽,体现其固本培元思想.
Abstract
Sun Yikui and Gong Tingxian both pay attention to the acquired spleen soil,anger damage to the liver,and elderly patients with physical deficiency in the treatment of tympanites.Spleen disease affects the liver,and liver disease affects the spleen.The liver and spleen are the key locations of tympanites.In terms of treatment,warming the spleen and lifting qi being the key.Gong Tingxian believed that the yin injury of the spleen and soil is of utmost importance.Therefore,based on the spleen and stomach,he often used the method of strengthening the spleen,promoting the flow of water,and widening the middle.The prescription is based on Sijunzi Decoction and Pingwei Powder;at the same time,attention should be paid to the use of drug formulations,such as decoction pills,powder pills,and processed drugs,which have the characteristics of simplicity and low cost.Sun Yikui believed that water deficiency was due to the deficiency cold of lower energizer.Therefore,he created Zhuangyuan Decoction,which should warm and replenish the lower original qi,making the fire qi abundant and the moisture evaporate.The stomach is warm,and the grain food is easy to dissolve,which is full and broad,reflecting the idea of consolidating the foundation and cultivating original qi.
