摘要
小儿胆道闭锁术后胆管炎属中医学"胎黄""黄疸""癥积"范畴.许华教授认为,胆道闭锁术后胆管炎病位在肝胆,与脾胃相关,基本病机为脾肾阳虚为本,感受湿邪为标,治疗原则为扶正祛邪、攻补兼施.从补肾、健脾中培本扶正,从祛除湿邪、调畅气机、活血化瘀、软坚散结4个方面治其标.方用茵陈四逆汤方随证加减,效果显著,为中医治疗小儿胆道闭锁术后胆管炎提供思路.
Abstract
Cholangitis after biliary atresia surgery in children belongs to the categories of"fetal jaundice","icterus"and"abdominal mass"in TCM.Professor Xu Hua believes that cholangitis after biliary atresia surgery is related to the liver and gallbladder,also the spleen and the stomach.The basic pathogenesis is based on spleen and kidney yang deficiency,with the perception of dampness as the superficiality.The treatment principle is to support the healthy qi and expel pathogens,and to simultaneously attack and supplement.The superficiality could be treated from the four aspects:tonifying the kidney and strengthening the spleen to cultivate the essence and nourish the body,eliminating dampness to regulate qi circulation and promote blood circulation and remove blood stasis,and softening and dispersing nodules.Modified Yinchen Sini Decoction is commonly used according to the syndromes,with remarkable efficacy,which would provide ideas for the treatment for cholangitis after biliary atresia surgery in children.
