目的 对麻杏石甘汤联合抗生素治疗社区获得性肺炎的疗效进行Meta分析。方法 检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、中国学术期刊数据库(万方数据)、中国生物医学文献数据库(CBM)、PubMed、Cochrane Library、Web of Science(WOS)核心合集建库至2023年2月6日收录的麻杏石甘汤治疗社区获得性肺炎的随机对照试验(RCT),采用RevMan5。3及Stata14。0软件进行Meta分析。结果 共纳入20项RCTs,总样本量1 443例。Meta分析结果显示,试验组临床总有效率[MD=-1。59,95%CI(-1。93,-1。25),P<0。000 01]高于对照组,试验组缩短退热时间[MD=-1。59,95%CI(-1。93,-1。25),P<0。000 01]、改善咳嗽[MD=-2。13,95%CI(-2。69,-1。58,),P<0。000 01]、改善咳痰[MD=-2。1,95%CI(-2。87,-1。34),P<0。000 01]、减少肺啰音持续时间[MD=-2。06,95%CI(-2。51,-1。61),P<0。000 01]、胸部X线吸收时间[MD=-4。73,95%CI(-6。48,-2。97),P<0。000 01]均优于对照组,治疗后血清WBC[MD=-1。22,95%CI(-1。85,-0。59),P=0。000 2]、CRP水平[MD=-13。75,95%CI(-21。63,-5。87),P=0。000 6]低于对照组;亚组分析所使用的抗生素中,莫西沙星或头孢哌酮/舒巴坦与麻杏石甘汤联合可增强疗效(P<0。05);联合治疗7 d或10 d可进一步缩短热程和咳嗽时间(P<0。05)。Egger's检验提示,研究可能存在一定发表偏倚。结论 麻杏石甘汤联合抗生素治疗社区获得性肺炎可提高抗生素疗效,联合药物可选择莫西沙星或头孢哌酮/舒巴坦,联合疗程可选择7 d或10 d,随疗程延长联合疗法优势更明显。
Meta-analysis on Maxing Shigan Decoction Combined with Antibiotics in the Treatment of Community-acquired Pneumonia
Objective To conduct a Meta-analysis on the efficacy of Maxing Shigan Decoction combined with antibiotics for the treatment of community-acquired pneumonia.Methods Randomized controlled trials(RCTs)of Maxing Shigan Decoction for the treatment of community-acquired pneumonia were retrieved from CNKI,VIP,Wanfang Data,CBM,PubMed,Cochrane Library and Web of Science(WOS)from the establishment of the databases to February 6,2023.RevMan 5.3 and StataMP 14.0 software were used for Meta-analysis.Results A total of 20 RCTs with 1 443 patients were included.Meta-analysis showed that the total clinical effective rate of the experimental group[MD=-1.59,95%CI(-1.93,-1.25),P<0.000 01]was higher than that of the control group.The experimental group shortened the antipyretic time[MD=-1.59,95%CI(-1.93,-1.25),P<0.000 01],improved cough[MD=-2.13,95%CI(-2.69,-1.58),P<0.000 01],improved sputum[MD=-2.1,95%CI(-2.87,-1.34),P<0.000 01],and reduced lung rale duration[MD=-2.06,95%CI(-2.51,-1.61),P<0.000 01],chest X-ray absorption time[MD=-4.73,95%CI(-6.48,-2.97),P<0.000 01]were all better than the control group.After treatment,serum WBC[MD=-1.22,95%CI(-1.85,-0.59),P=0.000 2],CRP levels[MD=-13.75,95%CI(-21.63,-5.87),P=0.000 6]were lower than the control group;among the antibiotics used in subgroup analysis,the combination of moxifloxacin or cefoperazone/sulbactam with Maxing Shigan Decoction could enhance the therapeutic effect(P<0.05);combination therapy for 7 or 10 days could further shorten the duration of fever and cough(P<0.05).The Egger's test suggested that there may be some publication bias in the study.Conclusion Maxing Shigan Decoction combined with antibiotics can improve the efficacy of antibiotics in the treatment of community-acquired pneumonia.Moxifloxacin or cefoperazone sulbactam can be selected as the combined drug,and the combined course of treatment can be selected for 7 or 10 days.Advantages of combined therapy are more obvious with the extension of treatment course.
