摘要
创面愈合是由于外伤、糖尿病溃疡、压疮、感染和术后创面不愈合等多种因素导致的如表皮、真皮、肌肉、筋膜等局部组织的损伤,其愈合过程是一个复杂且动态的程序,涉及到多种细胞反应和周围微环境的相互配合.目前,创面发病率呈逐年增长的趋势,造成了全球范围内巨大的社会和经济负担,老龄人口中发生的慢性损伤随着年龄的增长而逐年攀升.当前,修复创面的药物种类及方法多样,一定程度上减轻了患者的生理、心理和经济社会压力.中药单体及复方制剂修复创面具有独特的优势和力量,引起社会各界广泛的关注及应用,Wnt/β-catenin是调节细胞增殖、分化及凋亡,高度参与创面修复,是皮肤损伤修复过程中最重要的经典信号通路之一.Wnt/β-catenin信号通路通过中药单体及复方制剂降低机体创面中蛋白激酶、炎症反应,促进血管新生和表皮干细胞等表达,对创面愈合的治疗具有重要的参考价值.本文复习国内外相关文献,对Wnt/β-catenin信号通路与创面的关系及中药单体和中药复方通过调控该信号通路影响创面愈合的机制进行归纳总结,为中药治疗创面损伤提供新的思路和方向.
Abstract
Wound healing results from various factors such as trauma,diabetic ulcers,pressure sores,infections,and non-heal-ing surgical wounds,involving damage to local tissues such as the epidermis,dermis,muscles,and fascia.The healing process is complex and dynamic,involving multiple cellular responses and the cooperation of the surrounding microenvironment.Currently,the incidence of wounds is increasing annually,causing a significant social and economic burden globally,with chronic injuries in the aging population rising with age.Presently,the diversity of drugs and methods for wound repair has somewhat alleviated the physiological,psychological,and socio-economic pressures on patients.Traditional Chinese medicine,both in single-agent and compound formula-tions,has unique advantages and strengths in wound repair,attracting widespread attention and application from all sectors of society.The Wnt/β-catenin pathway,which regulates cell proliferation,differentiation,and apoptosis,is highly involved in wound repair and is one of the most important classical signaling pathways in the process of skin injury repair.Through traditional Chinese medicine,the Wnt/β-catenin signaling pathway reduces protein kinase and inflammatory responses in wounds,promotes angiogenesis,and the expression of epidermal stem cells,providing significant reference value for the treatment of wound healing.This article reviews the relevant domestic and international literature to summarize the relationship between the Wnt/β-catenin signaling pathway and wound healing and the mechanisms by which Chinese herbal monomers and compounds regulate this pathway to influence wound healing,offering new insights and directions for the treatment of wound injuries with traditional Chinese medicine.
