Objective To investigate specific markers for breast cancer metastasis in sentinel lymph nodes by studying the ex-pression of different cytokeratins in sentinel lymph nodes. Methods A total of 152 sentinel lymph nodes, received by Zhengzhou Sino-tec Medical Laboratory between September 2020 and December 2022,were included in the study. The inclusion criterion was cases requiring the determination of metastasis in sentinel lymph nodes, among which 27 cases were diagnosed as lymph node metastases.Intraoperative frozen sections from all cases underwent immunohistochemical staining with pan-cytokeratin (CK),CK8/18,CK7,and CK19.Results CK8/18 showed the strongest staining, followed by CK19 and pan-CK,with CK7 staining being weaker.Additionally, some sentinel lymph nodes with breast cancer metastasis exhibited negative CK7 staining.Notably,CK8/18 staining was observed in reticular fibroblasts of the stromal network in all lymph nodes. Conclusion CK19 is a reliable marker for assisting in the diagnosis of lymph node metastasis in breast cancer.Pan-CK can also be used as a marker for sentinel lymph node metastasis.However,CK8/18 and CK7 are not recommended as markers for breast cancer metastasis in sentinel lymph nodes.
