摘要
目的 调查在不同听力干预模式下听障患儿的生活质量差异.方法 回顾性选取2012年12月—2020年12月解放军总医院海南医院耳鼻咽喉头颈外科收治的行人工耳蜗植入术后开机、调机,并在海南省残疾人康复指导中心行听力学康复的先天性双耳极重度感音神经性耳聋的患儿101例,根据不同的听力干预方式分为双侧人工耳蜗植入组31例、双耳双模式组37例和单侧人工耳蜗植入组33例.按年龄段将患儿分为1~2岁段,3~4岁段,5~7岁段,>7岁段.采用本课题自行设计的一般资料调查问卷收集患儿人口学、听力学、家庭信息等.采用中文版人工耳蜗植入儿童家长观点调查问卷,比较3组患儿不同年龄段生活质量.结果 1~2岁段,3组植入效果比较,差异有统计学意义(F=3.681,P=0.039).3~4岁段,3组交流比较,差异有统计学意义(F=4.655,P=0.019).5~7岁段,3组植入效果比较,差异有统计学意义(F=5.851,P=0.009).>7岁段,3组交流、基本功能、教育、植入效果比较,差异均有统计学意义(P<0.05,P<0.01).结论 双侧人工耳蜗植入的重度-极重度感音神经性耳聋患儿在交流、基本功能、教育及植入效果评分表现良好,并且在7岁龄后优势更加明显.
Abstract
Objective To report quality of life outcomes in hearing impaired children using different strategies of hearing intervention.Methods This cross-sectional study included 101 children with congenital profound sensorineural deafness who received cochlear implants(CI)at the Hainan Hospital of the PLA General Hospital and underwent audio-logical rehabilitation at the Hainan Rehabilitation Guidance Center for the Disabled after CI activation and program-ming from December 2012 to December 2020.Quality of life was evaluated using ther Mandarin version of the Chil-dren with Cochlear Implants:Parental Perspectives.A general information questionnaire designed by the project was used to collect demographic,audiological and family history data.Quality of life outcomes were compared among chil-dren receiving bilateral CIs(n=31),bilateral CI + hearing aids(n=37)or unilateral CIs(n=33)in four age groups:i.e.1-2 years,3-4 years,5-7 years and over 7 years.Results Quality of life scores showed statistically significant difference among the three treatment modalities for children 1-2 years old(F=3.681,P=0.039)and those that were 5-7 years old(F =5.851,P=0.009).For children 3-4 years of age,statistically significant differences were noticed in the communication category among the three intervention modalities(F=4.655,P=0.019);and for those over 7 years of age,statistically sig-nificant differences were noticed in communication,essential functions,education and satisfaction with implantation categories(P<0.05 or P<0.01).Conclusion Children with severe to profound sensorineural hearing loss performed well in communication,essential functions,education and implant satisfaction after bilateral cochlear implantations,especial-ly after the age of 7 years.
