建立5G超声远程诊断系统质量控制标准研究

目的 建立5G超声远程诊断系统质量控制标准,规范会诊医师和基层医院医师的医疗行为,促进基层超声医师诊疗技术的提升,提高远程超声会诊诊断质量,并为5G远程医疗相关规范和指南的制定提供可借鉴和可参考的依据。方法 选取2021年10月—2023年5月应用海南省5G远程超声会诊系统进行超声会诊的223例病例纳入研究,对每例经远程超声会诊病例资料的超声扫查流程、检查切面、动态和静态图像采集及传输稳定性、测量切面是否标准、报告术语是否规范准确、危急值报告处置是否及时有效、检查阳性率等质控内容进行总结、分析和比对研究。结果 223例患者中,每例患者会诊时间为10~25min,平均(15。65±2。28)min;检查过程中,5G网络稳定流畅,无明显卡顿,会诊医师与视频端患者及医护人员均交流顺畅;大部分基层(邀请会诊端)医师检查操作规范性不够标准,经会诊医师指导后逐步标准化,但部分病例由于会诊医师无法感受到探头与患者体表接触的真实触感,而难以及时指导基层医师调整扫查方式,难以做到细微观察;检查切面、动态和静态图像采集及传输稳定性良好,但部分图像经压缩后质量略有下降,超声图像信息清晰度可,部分图像因操作端检查医师操作规范性、个人经验等原因而显示欠清晰。除此之外,在检查时由于受扫查速度和患者呼吸等的影响,部分检查切面图像出现延迟、分辨率降低等情况。会诊结果显示,223例患者检出率为92。83%,漏、误诊率为7。17%,阳性率为81。16%,阴性率为18。84%。结论 建立5G超声远程诊断系统质量控制标准,能够有效保障远程检查过程中网络的通畅以及会诊医师与视频端患者及基层医护人员交流顺畅,并确保检查流程规范、切面标准、动态和静态图像采集规范及传输稳定性,有助于提高检出率,减少漏诊和误诊发生。
Research on Establishing Quality Control Standard of 5G Ultrasonic Remote Diagnosis System
Objective To establish the quality control standard of 5G ultrasonic remote diagnosis system,standardize the medical behavior of consultation physicians and physicians in primary hospitals,promote the improvement of diagnosis and treatment techniques of primary sonographers,improve the quality of remote ultrasound consultation and diagnosis,and provide reference and reference basis for the formulation of 5G telemedicine related norms and guidelines.Methods From October 2021 to May 2023,223 cases of ultrasonic consultation using 5G remote ultrasonic consultation system in Hainan Province were selected and included in the study.The quality control contents such as ultrasonic scan process,inspection section,dynamic and static image acquisition and transmission stability,whether the measurement section is standard,whether the reporting term is standardized and accurate,whether the critical value report is timely and effective,and the positive rate of examination were summarized,analyzed and compared for each case of remote ultrasonic consultation data.Results Among 223 patients,the consultation time of each patient was 10-25 min,with an average of(15.65±2.28)min;During the examination,the 5G network was stable and smooth without obvious delay,and the consultation doctors communicated smoothly with the patients and medical staff at the video end.In terms of the operation of doctors at the grass-roots end(inviting consultation end),most of the basic doctors'inspection operation is not standard enough,and it is gradually standardized after the guidance of the consultant.However,in some cases,because the consultant can't feel the real touch of the probe and the patient's body surface,it is difficult to guide the primary doctors to adjust the scanning method in time,and it is difficult to achieve subtle observation;Inspection section,dynamic and static image acquisition and transmission stability is good,but the quality of some images is slightly decreased after compression,the clarity of audio image information can be,and some images are not clear due to the operation of the inspection doctor at the operation end,personal experience and other reasons.In addition,due to the impact of scanning speed and patient breathing,some images of the inspection section have delays and reduced resolution.The results of consultation showed that the detection rate of 223 patients was 92.83%,the rate of missing and misdiagnosis was 7.17%,the positive rate was 81.16%,and the negative rate was 18.84%.Conclusion The establishment of quality control standards for 5G ultrasonic remote diagnosis system can effectively ensure the unobstructed of the network during remote examination,the smooth communication between the consultation physician and the video patients and the primary medical staff,and ensure the inspection process specifications,section standards,dynamic and static image acquisition specifications and transmission stability,which is helpful to improve the detection rate and reduce the incidence of missed diagnosis and misdiagnosis.
