Comparison and Deviation Evaluation Between Arterial Blood Gas Analysis and Venous Blood Analysis of Elderly Patients
Objective To compare and evaluate deviation between arterial blood gas analysis and seven identical items of venous blood(hemoglobin,hematocrit,potassium,sodium,chloride,calcium,glucose)of elderly patients.Methods The paper chose 110 elderly patients,aged(50-95),with an average age of 77.5,in Internal Medicine Department of Guangzhou City Dongsheng Hospital from October 2022 to January 2023 randomly,simultaneously drew arterial and venous blood at a certain time during hospitalization,and carried on arterial blood gas analysis,blood routine,biochemical electrolyte and glucose concentration testing.Relative deviation or systematic error at medical decision level of mean measurement of experimental and comparative system was analyzed with paired t-tests and regression statistics.Clinical acceptable performance of different detection systems was determined based on 1/2 of allowable error range for room quality evaluation specified in the US Clinical Laboratory Amendment Regulation(CLIA88).Data linear regression statistical analysis was carried out to clarify correlation coefficient r and regression equation.Results Hemoglobin,hematocrit,potassium,sodium,chloride,calcium,and glucose difference showed statistical significance(P<0.05).All seven items were positively correlated,arterial blood gas had higher levels of hemoglobin,hematocrit,chlorine,and glucose than venous blood.Corresponding correlation coefficients r were 0.957,0.930,0.971,and 0.993(P<0.05)respectively.Potassium,sodium,and calcium levels in arterial blood gas were lower than venous blood,corresponding correlation coefficients r were 0.970,0.948(P<0.05),and 0.706(P<0.05)respectively.According to 1/2CLIA'88 and CAP room quality assessment,with judgment standard of allowable error,deviation SE of potassium,sodium,chloride,calcium,and glucose could be clinically accepted based on judgment standard of being≤1/2TEa.Conclusion Deviation SE between arterial blood gas analysis and venous serum potassium,sodium,chloride,calcium,and glucose is≤1/2TEa,which is acceptable.However,deviation SE of arterial blood gas hemoglobin and hematocrit is>1/2TEa,which can be estimated through regression equations and has certain reference value.
