摘要
目的 研究高渗盐液治疗急诊重度脑外伤并休克的效果.方法 抽选取2022年6月—2023年6月本院收治的急诊重度脑外伤并休克的104例患者为研究对象,根据住院号尾数奇偶性将其分为观察组和对照组,每组52例.对照组采用甘露醇注射液、林格氏液治疗,观察组采用高渗盐液治疗.比较两组患者临床效果、生命体征情况、格拉斯哥昏迷量表(GCS)评分、生活质量评定量表(QOL)评分.结果 观察组临床有效率明显高于对照组(P＜0.05);观察组死亡率明显低于对照组(P＜0.05).与治疗30min后相比较,治疗2h后观察组与对照组患者各项生命体征均好转,而观察组RR、HR、ICP、CPP、MAP、UPD比对照组更优(P＜0.05).治疗后,观察组GCS评分比对照组更高(P＜0.05).治疗后,观察组QOL评分比对照组更高(P＜0.05).结论 采取高渗盐液治疗急诊重度脑外伤并休克具有较好疗效,能提升患者的抢救成功率,加速患者恢复,临床价值高.
Abstract
Objective To observe effect of hypertonic saline solution for severe traumatic brain injury with shock in emergency department and analyze effect of QOL scores.Methods The paper chose 104 patients diagnosed with severe traumatic brain injury and shock in emergency department of our hospital from June 2022 to June 2023 as research subjects,and divided them into observation group and control group based on parity of hospitalization numbers,with 52 cases in each group.Control group was treated with mannitol injection and Ringer's solution,while observation group with hypertonic saline solution.Clinical effect,vital signs,GCS scores,and QOL scores were compared between two groups.Results Clinical effective rate of observation group was significantly higher than control group,difference was statistically significant(P<0.05).Mortality rate of observation group was significantly lower than control group,difference was statistically significant(P<0.05).Compared with condition of 30 minutes after treatment,vital signs in observation group and control group were better after 2 hours of treatment.However,observation group had better RR,HR,ICP,CPP,MAP,and UPD than control group,(P<0.05).After treatment,observation group had higher GCS scores than control group(P<0.05).After treatment,observation group had higher QOL scores than control group(P<0.05).Conclusion Hypertonic saline solution can achieve high curative effect for severe traumatic brain injury with shock in emergency department,can improve success rate of rescue,accelerate recovery of patients,which has high clinical value.
