Application Analysis of Personalized Family Doctor Contract Service Management Model in Community Management of Type 2 Diabetes Patients
Objective To explore application effect of personalized family doctor contract service management model in management of type 2 diabetes patients in community.Methods The paper chose 100 patients with type 2 diabetes managed in community from June 2022 to June 2023 as research objects,and divided them into two groups randomly,50 cases in each group.Control group was treated with routine management,while observation group with personalized family doctor contract service management.Blood glucose related indicators,incidence of hypoglycemia,blood glucose variability,and health behavior were compared between two groups before and after intervention.Results After intervention,observation group had better blood glucose related indicators than control group,incidence of hypoglycemia was lower(P<0.05).After intervention,blood glucose variability related indicators in observation group were better than control group(P<0.05).Health behaviors in observation group was better than control group(P<0.05).Conclusion Personalized family doctor contract service management model is effective for community type 2 diabetes patients,can improve blood sugar control effect and help them form healthy behavior habits,which is worthy of application.
