Value Analysis of Color Doppler Ultrasound in Diagnosing Endometriosis at Site of Abdominal Wall Scar
Objective To evaluate application of color Doppler ultrasound in diagnosis of endometriosis in abdominal wall scars. Methods The paper reviewed and analyzed clinical data from 76 cases highly suspected endometriosis in abdominal wall scar in our hospital, examined with color Doppler ultrasound and lower abdominal CT according to standard examination procedure. After completing the examinations, scheduled laparoscopic and pathological examinations were carried out to confirm the diagnosis.Reevaluation and clear diagnosis was completed with single blind method, and number of diagnoses with two methods was counted.Number of confirmed, missed or misdiagnosed cases, diagnostic value, and consistency between two methods and diagnostic gold standard was analyzed with diagnostic criteria of laparoscopic examination and pathology. Results There was no statistical significance in comparison of missed diagnosis rate and misdiagnosis rate between two methods(P>0.05),while accuracy of CT examination(82.89%)was lower than ultrasound(93.42%),and the comparison was statistically significant(P<0.05).Comparison of positive predictive value, negative predictive value, sensitivity, and specificity between two methods was not statistically significant(P>0.05).According to Kappa consistency analysis,CT examination had strong consistency with the diagnostic gold standard(Kappa=0.734,P<0.05),while ultrasound examination showed stronger consistency with diagnostic gold standard(Kappa=0.812,P<0.05).Conclusion Color Doppler ultrasound can achieve significant application value in diagnosing endometriosis in abdominal wall scar, can improve diagnostic accuracy.During practical application,a reasonable examination method can be selected based on examination indications of patients,or appropriate combination can be used to further improve diagnostic value.
