Effect of Thrombus Aspiration Combined with Intracoronary Injection of Recombinant Human Prourokinase on Improving Myocardial Perfusion Levels of Patients with Acute ST Segment Elevation Myocardial Infarction
Objective To explore effect of thrombus aspiration combined with intracoronary injection of recombinant human prourokinase on improving myocardial perfusion levels of patients with acute ST segment elevation myocardial infarction.Methods The paper chose 80 patients with acute ST segment elevation myocardial infarction(STEMI)in our hospital from June 2020 to December 2022,and divided them into two groups randomly.Control group was treated with thrombus aspiration only,while observation group with thrombus aspiration combined with intracoronary injection of recombinant human prourokinase.Curative effect,postoperative blood flow perfusion indicators,myocardial injury markers and cardiac function indicators,MACE,and bleeding rate were compared between two groups.Results There was no significant difference in preoperative TIMI grading between two groups(P>0.05),proportion of grade 3 in immediate postoperative TIMI grading in observation group was significantly higher than control group(P<0.05).The proportion of MBG grade 3 and the rate of ST segment regression≥50%in observation group were higher than control group on the 1st day after surgery,incidence of postoperative NRP was lower than control group(P<0.05).Postoperative BNP,Mb,CK-MB,and cTn Ⅰ in observation group was lower than control group,while LVEF was higher than control group(P<0.05).The MACE of recurrent angina pectoris,recurrent STEMI,stent thrombosis,target vessel reconstruction,and sudden cardiac death in observation group was significantly lower than control group(P<0.05),and incidence of TIMI major bleeding and TIMI minor bleeding between two groups was comparable(P>0.05).Conclusion Combination of thrombus aspiration and intracoronary injection of recombinant human urokinase can achieve significant effect for STEMI,can restore myocardial blood flow perfusion,reduce NRP rate,improve heart function,and alleviate myocardial injury effectively.Moreover,it has low MACE and lower bleeding risk with high safety.
