摘要
目的 探讨内镜逆行阑尾炎治疗术(endoscopic retrograde appendicitis therapy,ERAT)治疗成人非复杂性阑尾炎的临床价值.方法 选择2020年10月—2022年9月在渭南市中心医院就诊并诊断为非复杂性阑尾炎的患者60例,根据患者治疗方式分为ERAT组和腹腔镜阑尾切除术(laparoscopic appendectomy,LA)组,每组30例,分别进行ERAT术或LA术.记录患者治疗成功率,术前和术后2h、24h VAS评分,住院天数,住院总费用,患者满意度和术后不良事件.结果 ERAT组中有29例(96.7%)患者治疗成功,而LA组中有30例(100.0%)患者治疗成功,两组治疗成功率差异无统计学意义(P>0.99);与术前相比,两组患者术后2h和24h VAS评分均显著下降,且ERAT组下降幅度大于LA组(P<0.05);ERAT组患者住院天数和总费用均显著低于LA组(P<0.05);两组患者满意度对比,差异无统计学意义(P<0.05).结论 ERAT是治疗成人非复杂性阑尾炎的一种安全、有效的方法,值得在临床推广应用.
Abstract
Objective To explore clinical value of endoscopic retrograde appendicitis therapy(ERAT)in treatment of adult non complex appendicitis.Methods The paper chose 60 patients,including 28 males and 32 females,aged≥18,diagnosed with non complex appendicitis in Weinan City Central Hospital from October 2020 to September 2022,and divided them into ERAT group and laparoscopic appendectomy(LA)group according to treatment method of patients,with 30 cases in each group,treated with ERAT or LA respectively.Success rate of treatment,preoperative and postoperative VAS scores at 2 and 24 hours,length of stay,total hospitalization cost,satisfaction degree,and postoperative adverse events were recorded.Results 29 cases(96.7%)in ERAT group were successfully treated,while 30 cases(100%)in LA group were successfully treated.There was no statistically significant difference in treatment success rates between two groups(P>0.99).VAS scores at 2 and 24 hours after operation in two groups were significantly were than before operation,ERAT group had greater decrease than LA group(P<0.05).Length of hospital stay and total cost of ERAT group was significantly lower than LA group(P<0.05).There was no statistically significant difference in satisfaction degree between two groups(P<0.05).Conclusion ERAT is a safe and effective method for adult non complex appendicitis,which is worthy of clinical promotion and application.
