目的 探讨术后无管化在肾上腺切除术后患者快速康复中的应用效果.方法 选取2021年10月—2022年10月浙江省人民医院泌尿外科收治的104例行单侧肾上腺全切患者为研究对象,对其临床资料进行回顾性分析,根据是否留置腹腔引流管分成对照组(60例,常规留置1根腹腔引流管)和实验组(44例,不留置引流管).两组均实施基于加速康复外科理念(enhanced recovery after surgery,ERAS)的临床护理,比较两组患者的手术时间、术中出血量、术后绝对卧床时间、首次进食时间、术后住院天数、术后疼痛、术后并发症.结果 实验组患者手术时间、术后绝对卧床时间、住院时间均短于对照组,差异有统计学意义(P<0.05).术后2h疼痛情况比较,实验组低于对照组,差异有统计学意义(P<0.05).实验组并发症总发生率低于对照组(P<0.05).结论 单侧肾上腺全切术后,在充分止血前提下,不留置腹腔引流管有利于加速患者术后恢复,减少并发症的发生,缩短术后住院时间,减轻患者疼痛.
Application of Ductectomy for Rapid Recovery of Patients After Adrenalectomy
Objective To explore application effect of postoperative ductectomy in rapid recovery of patients after adrenalectomy. Methods With study period from October 2021 to October 2022,104 patients with unilateral total adrenalectomy in the department of urology of Zhejiang Provincial People's Hospital were chosen as study subjects,the clinical data were reviewed and analyzed,who were divided into the control group (60 cases,treated with routine placement of one abdominal drainage tube) and the experimental group (44 cases,without placement of drainage tubes) based on placement of abdominal drainage tubes. Both groups were treated with clinical nursing based on the enhanced recovery after surgery (ERAS) concept,and surgical time,intraoperative blood loss,absolute bed rest time,first meal time,length of stay,postoperative pains,and postoperative complications were compared between two groups. Results The surgical time,absolute bed rest time,and length of stay of the experimental group were shorter than the control group,difference was statistically significant (P<0.05). The comparison showed,pain levels 2 hours after surgery of the experimental group were lower than the control group,and difference between two groups was statistically significant (P<0.05). Incidence of complications was lower than the control group (P<0.05). Conclusion After unilateral total adrenalectomy,under the premise of sufficient hemostasis,no abdominal drainage pathway is beneficial for postoperative recovery,reducing incidence of complications,shortening postoperative length of stay,and alleviating pains of patients.
