Effect Analysis of DRG on Refined Management of Orthopedic Inpatients in Public Hospitals
Objective To explore the effect of disease diagnosis related grouping (DRG) on refined management of orthopedic inpatients in public hospitals. Methods The paper collected 3160 discharged patients in our orthopedic department in the DRG group from January 2021 to May 2023 as study subjects. Among them,1580 discharged orthopedic patients from January 2021 to February 2022 were included in the reference group,treated with routine management instead of refined management of DRG grouping,and 1580 orthopedic discharged patients from March 2022 to May 2023 in the intervention group,with refined management of DRG grouping. Medical service evaluation indicators (DRG grouping number,total weight,case combination index CMI,time consumption index,cost consumption index,low-risk mortality rate) and hospitalization related indicators (average hospitalization cost per visit,drug cost per visit,average length of stay) were compared between two groups. Overall situation of DRG overspending cases was compared between two groups (overspending rate,overspending total hospitalization expenses,overspending total drug expenses). Satisfaction degree with medical services was compared between two groups (medical technology,physician rounds,treatment effect,and clean medical practice;nursing technology,introduction of precautions,service attitude,and medical fees). Results The observation group had lower medical service evaluation indicators of case combination index (CMI),time consumption index,and cost consumption index than the reference group (P<0.05). The hospitalization related indicators of the observation group,were lower than the reference group,including average hospitalization costs,medication costs,and average daily hospitalization days,(P<0.05). The overspending rate of the observation group was 29.75%,significantly lower than 41.65% in the reference group (P<0.05). However,the decrease in total hospitalization expenses and total drug expenses in the observation group was not significant compared with the reference group (P>0.05). Satisfaction degree of inpatients in the observation group was higher than the reference group in eight aspects,inclufing medical technology,physician rounds,curative effect,honest medical practice and nursing techniques,introduction of precautions,service attitude and medical fees (P<0.05). Conclusion Refined management based on DRG can improve indicators of the medical service system significantly,reduce hospitalization and drug costs,shorten length of stay,reduce overspending rate,and improve satisfaction degree of orthopedic inpatients with medical services,which is worthy of promotion and deep exploration.
