摘要
目的 检索、评价并汇总动脉瘤性蛛网膜下腔出血患者深静脉血栓预防的最佳证据.方法 计算机系统检索国内外循证数据库、综合数据库、专业协会网站中关于动脉瘤性蛛网膜下腔出血患者深静脉血栓预防的指南、证据总结、专家共识、系统评价、临床决策.检索时限为2011年1月—2023年3月.由4名研究者独立进行文献质量评价,由2名研究者对符合质量评价标准的文献进行证据提取和总结.结果 总共纳入14篇文献,包括2篇临床决策、5篇指南、2篇系统评价、5篇专家共识.最终从危险因素评估、基础预防、机械预防、药物预防、神经系统并发症预防、知情同意、突发动脉瘤破裂的急救7个方面总结形成31条证据.结论 动脉瘤性蛛网膜下腔出血患者深静脉血栓预防最佳证据可为临床医护人员提供循证依据,建议医护人员根据临床实际采纳证据,以降低动脉瘤性蛛网膜下腔出血患者深静脉血栓的发生率.
Abstract
Objective To retrieve,evaluate and summarize the best evidence for the prevention of deep vein thrombosis in patients with aneurysmal subarachnoid hemorrhage.Methods Guidelines,evidence summaries,expert consensus,systematic reviews and clinical decisions on the prevention of deep vein thrombosis in patients with aneurysmal subarachnoid hemorrhage were retrieved from evidence-based databases,comprehensive databases and professional association websites.The search period was from January 2011 to March 2023.Four researchers independently evaluated the quality of the literature,and two researchers extracted and summarized the evidence that met the quality standards.Results A total of 14 articles were included,including 2 clinical decisions,5 guidelines,2 systematic reviews,and 5 expert consensuses.Finally,31 pieces of evidence were summarized from 7 aspects,including risk factor assessment,basic prevention,mechanical prevention,drug prevention,prevention of neurological complications,informed consent,and first aid for sudden aneurysm rupture.Conclusion The best evidence for prevention of deep vein thrombosis in patients with aneurysmal subarachnoid hemorrhage can provide evidence-based basis for clinical medical staff.It is recommended that medical staff adopt the evidence according to clinical practice to reduce the incidence of deep vein thrombosis in patients with aneurysmal subarachnoid hemorrhage.
