左心室辅助装置植入患者围手术期行家庭参与式护理的实践

摘要
总结6例左心室辅助装置植入患者围手术期家庭参与式护理经验.护理要点:组建专案小组,选择合适的家庭照护责任人参与术前状态优化和心理支持、术后危重期鼓励家属参与ICU谵妄预防;出院准备期以家庭照护培训为重点,分阶段完成机械辅助装置管理、抗凝管理、症状管理、感染防控、日常生活管理五大模块的培训内容.6例患者皆顺利完成手术,家庭照护责任人全程参与围手术期护理,出院前掌握目标知识和技能,出院后家庭照护效果满意,居家期间随访无仪器设备故障及其他并发症,无非计划二次入院.
Abstract
To summarize the perioperative care experience of 6 patients with left ventricular assist device implantation based on family integrated care.The main points of care were implemented.We formed a project team to select appropriate family caregivers to participate in preoperative state optimization and psychological support,and encourage family members to participate in ICU delirium prevention.During the discharge planning period,the focus was on the training of family caregivers,and the training content of five modules were completed in phases,including the management of mechanical assistive devices,anticoagulation management,symptom management,infection prevention and control and the management of daily life.All patients successfully completed the surgery,and family caregivers were fully involved in perioperative care and mastered the target knowledge and skills before discharge,and were satisfied with family care after discharge,with no instrument and equipment failures and other complications during the follow-up at home,and no unplanned secondary admissions.
