摘要

目的 构建院前急救输血全流程信息化质量管理方案,评价其应用效果.方法 成立多学科研究团队,基于无线射频识别技术、低温储藏技术与物联网技术,构建院前急救输血全流程信息化质量管理方案,对院前急救输血实施过程进行全流程信息化管理和追溯,并进行调控与动态评价.将院前急救输血全流程信息化质量管理方案实施前(2020年1月-2021年12月)与实施后(2022年1月-2023年12月)的数据进行比较,评价其在院前急救输血中的效果.结果 院前急救输血全流程信息化质量管理方案实施后,急救用血等待时间由53.28 min缩短至19.70 min(P<0.001),取血消耗时间由15.08 min缩短至8.76 min(P<0.001),取血核对时间由2.54 min缩短至1.99 min(P<0.001),平均住院天数从15.80 d缩短至9.68 d(P<0.001),患者抢救成功率从91.5%提高至99.0%(P<0.05),并发症发生率从9.4%降至1.0%(P<0.05),血液浪费率从39.6%降至0(P<0.001).结论 院前急救输血全流程信息化质量管理方案可显著缩短患者用血时间,提高救治效率,保障院前急救患者的用血安全.

Abstract

Objective To develop and explore the application effects of information quality management plan for the whole process of pre-hospital emergency blood transfusion.Methods A multidisciplinary research team was established,our hospital used radio frequency identification(RFID)technology,low temperature storage technology and Intemet of Things technology to jointly construct an information quality management plan for the whole process of pre-hospital emergency blood transfusion,to carry out the whole process information management and traceability of the implementation process of pre-hospital emergency blood transfusion,and conduct dynamic regulation and evaluation.We selected and compared the data before the application of prehospital emergency blood transfusion whole process information quality management plan(January 2020 to December 2021)and after the application(January 2022 to December 2023),to evaluate the effect of this plan in pre-hospital emergency blood transfusion.Results After the implementation of the information quality management plan for the whole process of pre-hospital emergency blood transfusion,the waiting time for emergency blood decreased from 53.28 minutes to 19.70 minutes(P<0.001),the blood collection time decreased from 15.08 minutes to 8.76 minutes(P<0.001),the average hospital stay decreased from 15.80 days to 9.68 days(P<0.001),the rescue success rate increased from 91.5%to 99%(P<0.05),and the incidence of complications decreased from 9.4%to 1%(P<0.05),the rate of blood waste decreased from 39.6%to 0(P<0.001).Conclusion The information quality management plan for the whole process of pre-hospital emergency blood transfusion can significantly shorten the time of blood use,improve the treatment efficiency,and ensure the safety of blood use in pre-hospital emergency patients.
