2型糖尿病合并牙周炎老年患者血清炎症及氧化应激水平与牙周参数的相关性

摘要
目的 探讨2型糖尿病(T2DM)合并牙周炎老年患者血清糖基化终末产物(AGE)、C反应蛋白(CRP)、丙二醛(MDA)水平与牙周参数的相关性.方法 选择 2021 年 6 月至 2022 年 10 月濮阳市人民医院口腔科T2DM合并牙周炎老年患者45例作为T2DM牙周炎组,并选择同期老年健康体检者45例作为对照组,牙周炎老年患者45例作为单纯牙周炎组.比较入选患者一般资料、牙周参数[探诊深度(PD)、龈沟出血指数(SBI)]、血清因子指标(AGE、CRP、MDA),采用Pearson分析血清AGE、CRP、MDA水平与牙周参数的相关性.结果 单纯牙周炎组、T2DM牙周炎组PD、SBI高于对照组,T2DM牙周炎组PD、SBI高于单纯牙周炎组(P<0.05).单纯牙周炎组、T2DM牙周炎组血清AGE、CRP、MDA水平高于对照组,T2DM牙周炎组血清AGE、CRP、MDA水平高于单纯牙周炎组(P<0.05).Pearson相关性显示,血清AGE、CRP与PD之间存在强相关性(r=0.611,P<0.001;r=0.646,P<0.001),血清MDA与PD之间存在中等程度相关(r=0.554,P<0.001);血清AGE与SBI之间存在中等程度相关(r=0.575,P<0.001),血清CRP、MDA与SBI之间存在强相关性(r=0.691,P<0.001;r=0.689,P<0.001).结论 T2DM合并牙周炎老年患者血清AGE、CRP、MDA水平明显升高,且上述指标水平与牙周参数PD、SBI呈正相关关系.
Abstract
Objective To investigate the correlation between the levels of serum advanced glycation end products(AGE),c-reactive protein(CRP),malondialdehyde(MDA)and periodontal parameters in elderly patients with type 2 diabetes mellitus(T2DM)complicated with periodontitis.Methods 45 elderly patients with T2DM combined with periodontitis in our hospital from June 2021 to October 2022 were selected as T2DM periodontitis group,.45 healthy elderly patients with periodontitis were selected as control group,and 45 elderly patients with periodontitis were selected as simple periodontitis group.General data,periodontal parameters[probing depth(PD),sulcus bleeding index(SBI)]and serum factor indexes(AGE,CRP,MDA)of the enrolled patients were compared.The correlation between serum AGE,CRP,MDA levels and periodontal parameters was analyzed by Pearson.Results PD and SBI in simple periodontitis group and T2DM periodontitis group were higher than those in control group,and PD and SBI in T2DM periodontitis group were higher than those.in simple periodontitis group(P<0.05).The levels of serum AGE,CRP and MDA levels in simple periodontitis group and T2DM periodontitis group were higher than those in control group,and the levels of serum AGE,CRP and MDA levels in T2DM periodontitis group were higher than those in simple periodontitis group(P<0.05).Pearson correlation showed that serum AGE,CRP and PD were strongly correlated(r=0.611,P<0.001;r=0.646,P<0.001),serum MDA was moderately correlated with PD(r=0.554,P<0.001).Serum AGE was moderately correlated with SBI(r=0.575,P<0.001),and serum CRP,MDA and SBI were strongly correlated(r=0.691,P<0.001;r=0..689,P<0.001).Conclusions Serum AGE,CRP and MDA levels were significantly increased in elderly patients with T2DM and periodontitis,and the above indexes were positively correlated with periodontal parameters PD and SBI.
