Association analysis between postoperative pain status and preoperative pain sensitivity and dental fear in elderly patients with dental implant surgery
Objective To investigate association analysis between postoperative pain status and preoperative pain sensitivity and dental fear in elderly patients with dental implant surgery. Methods 125 elderly patients undergoing dental implant surgery were recruited at Stomatological Hospital,Southern Medical University from July 2023 to February 2024. The Pain Sensitivity Questionnaire and Dental Fear Survey were completed preoperatively,and the Faces Pain Scale-Revised and the Brief Pain Inventory were used to assess postoperative pain intensity and pain interference. Patients were divided into mild (1-3),moderate (4-6) and severe (7-10) pain groups according to the worst pain scores on the first postoperative day. Logistic regression analysis and Spearman' s correlation coefficient were used to analyze factors influencing postoperative pain,and the association between postoperative pain and preoperative pain sensitivity and dental fear,respectively. Results Current pain intensity at 6 hours in patients after dental implant surgery was 4 (2,6),and worst pain intensity was 6 (4,8) on the first postoperative day,then decreased from the second postoperative day,with almost no pain on the seventh postoperative day. The pain interference on patients' sleep on the first postoperative day was 2 (0,5). On the first postoperative day,percentage of patients with mild,moderate,and severe worst pain accounted for 13.6%,47.2%,and 39.2%,respectively. Statistically significant differences among three groups in pain sensitivity,number of implants placed,surgical approach,and oral analgesic use were observed (P<0.05). Patients with implants of two and over and oral analgesic use were factors influencing postoperative pain intensity (P<0.05). Preoperative pain sensitivity and dental fear were significantly associated with pain interference after surgery (P<0.05). Conclusions Elderly patients with dental implant surgery experience mild to severe postoperative pain,with higher levels of worst pain intensity and interference on sleep on the first postoperative day,which is influenced by number of implants placed and oral analgesic use. Attention needs to be paid to those with higher preoperative pain sensitivity and dental fear,to reduce the impact of postoperative pain.
