Investigation and analysis of the implementation of hierarchical medical system for oncology in China
Objective:To investigate the current implementation of hierarchical medical system for oncology in China and provide evidence for the rational allocation of oncology diagnosis and treatment resources.Methods:A survey questionnaire based on the core components of the hierarchical medical system was designed and distributed through the National Cancer Standardized Diagnosis and Treatment Network.Collected responses were statistically analyzed.Results:A total of 769 questionnaires were collected from 346 hospitals.Among these,284 hospitals conducted bidirectional referrals,24 only referred patients upward,23 only referred patients downward,and 15 did not perform any referrals.Regionally,the proportion of hospitals not conducting referrals was 8.1%in eastern regions,0.8%in central regions,and 1.6%in western regions(P<0.01).By hospital type,85.4%of oncology-specialized hospitals used systematic referrals compared to 52.8%of non-specialized hospitals(P<0.01).By hospital grade,58.5%of tertiary hospitals utilized systematic referrals compared to 30.0%of secondary hospitals(P<0.05).Conclusion:The overall implementation of hierarchical medical system for oncology is relatively effective;however,disparities between regions and low levels of informatized referrals persist,requiring further improvement.
