摘要
目的 对我国脑肿瘤患儿的健康相关生活质量(heath-related quality life,HRQOL)进行初步调查,并对HRQOL的可能影响因素进行分析,为制定措施改善患儿的预后以及生活质量提供新的依据.方法 本研究采取便利抽样,向西京医院神经外科2010.01~2021.08收治的部分脑肿瘤患儿及家长发放一般资料调查问卷及简体中文版PedsQLTM脑肿瘤模块量表(2~18岁).通过单因素及多因素分析得出可能影响量表得分的因素.结果 患儿的"认知问题"维度得分在接受治疗情况和上学情况间的差异有统计学意义(P<0.05)."疼痛与痛苦"维度得分在医学诊断、脑室腹腔引流管、上学情况、肿瘤位置和家长工作状态间的差异有统计学意义(P<0.05)."运动与平衡"维度得分在医学诊断、上学情况、肿瘤位置和家长工作状态间的差异有统计学意义(P<0.05)."程序性焦虑"维度得分仅在脑室腹腔引流管间的差异有统计学意义(P<0.05)."恶心"维度得分仅在接受治疗情况间的差异有统计学意义(P<0.05).患儿的"担心"维度得分在患儿性别、年龄、家长年龄间的差异有统计学意义(P<0.05).量表总得分在接受治疗情况、上学情况、肿瘤位置、家长工作状态、家庭经济情况间的差异有统计学意义(P<0.05).结论 本研究初步调查了我国脑肿瘤患儿HRQOL现状,发现我国患儿的HRQOL与接受治疗情况、上学情况、肿瘤位置、家长工作状态、家庭经济情况相关,需引起关注.
Abstract
Objective This study provided an initial exploration of the health-related quality of life(HRQOL)of pediatric brain tumour patients in China and examined the variables that might influence HRQOL.The results were expected to provide innovative insights to the strategies to improve clinical outcomes and quality of life for this patient population.Methods In this study,convenience sampling was employed to distribute a general information questionnaire and the Chinese version of the PedsQLTM Brain Tumor Module(for ages 2~18)to a subset of pediatric brain tumor patients and their parents admitted to the Neurosurgical Department of Xijing Hospital from January 2010 to August 2021.Univariate and multivariate analyses were conducted to identify factors potentially influencing the scores on the questionnaire.Results Scores on the"Cognitive Problems"dimension of the questionnaire showed statistically significant differences based on the treatment status and school attendance of the patients(P<0.05).The"Pain and Suffering"dimension scores revealed statistically significant differences based on medical diagnosis,presence of ventriculoperitoneal shunt,school attendance,tumor location,and parents'employment status(P<0.05).Scores on the"Mobility and Balance"dimension were significantly different based on medical diagnosis,school attendance,tumor location,and parents'employment status(P<0.05).The"Procedural Anxiety"dimension scores only showed statistically significant differences related to the presence of a ventriculoperitoneal shunt(P<0.05).The"Nausea"dimension scores showed statistically significant differences solely in relation to the treatment status(P<0.05).The"Worry"dimension scores of the patients showed statistically significant differences based on the patients'gender,age,and parents'age(P<0.05).The total scale scores displayed statistically significant differences according to treatment status,school attendance,tumor location,parents'employment status,and family economic status(P<0.05).Conclusion This research undertook an initial exploration of the health-related quality of life(HRQOL)in pediatric patients with brain tumors in China.The results suggest that HRQOL in these children was influenced by factors such as treatment regimen,school participation,tumor site,parents'employment,and family financial status,highlighting that the above aspects deserved increased consideration and potential intervention.
