慢性肾功能衰竭患者合并脑出血的预后研究:一项单中心回顾性PSM研究

摘要
目的 探讨慢性肾功能衰竭对脑出血患者预后的影响.方法 回顾性分析2019年1月到2023年9月期间在我院住院的自发性脑出血患者的临床资料,对于患者预后状态采用二分类处理,回顾性分析预后相关因素.对于数据不平衡的问题,采用倾向得分匹配(PSM)方法进行调整后Logistic回归分析.结果 研究共纳入1102例脑出血患者,其中慢性肾功能衰竭患者30例(2.7%),脑出血期间坚持透析患者13例(1.2%).与肾功能衰竭相关的混杂因素包括糖尿病、格拉斯哥昏迷评分法(GCS)评分、充血性心功能衰竭、心率、收缩压、K+浓度、入院随机血糖等.进行平衡配对后,慢性肾功能衰竭患者的预后显著差于无肾功能衰竭的患者.结论 与不伴有慢性肾功能衰竭的脑出血患者相比,存在慢性肾功能衰竭的脑出血患者死亡率更高,预后更差.
Abstract
Objective To explore the impact of chronic renal failure on the prognosis of patients with intracerebral hemorrhage(ICH).Methods The clinical data of patients with spontaneous ICH hospitalized in our hospital from January 2019 to September 2023 were retrospectively analyzed,and the prognosis status of patients was classified separately,and the prognostic factors were retrospectively analyzed.For the problem of data imbalance,an adjusted Logistic regression analysis was performed using the PSM method.Results A total of 1102 patients with ICH were included in the study,including 30(2.7%)with chronic renal failure and 13(1.2%)with dialysis during the period.Confounding factors associated with renal failure included diabetes mellitus,GCS,congestive heart failure,heart rate,systolic pressure,potassium,random blood glucose levels,etc.After balanced pairing,the prognosis of patients with chronic renal failure was significantly worse than that of those without renal failure.Conclusion Compared with those without chronic renal failure,those with chronic renal failure have higher mortality rate and worse prognosis.
