Diagnosis and treatment of bipolar clavicle injury
Objective To investigate the mechanism and clinical outcome of bipolar clavicle injury.Methods A retrospective analysis was conducted on 8 patients who underwent bipolar clavicle injury treatment in our hospital from January 2013 to January 2023,all of whom had one or more other injuries.According to the situation of clavicle injury,there were 2 cases of bipolar dislocation,2 cases of acromioclavicular joint dislocation combined with proximal clavicle fracture,1 case of distal clavicle fracture combined with thoracoclavicular joint dislocation,and 3 cases of bipolar fracture.During the follow-up period,the fracture healing,postoperative complications,and range of motion of the affected limb's shoulder joint were observed.Constant score,ASES score,and DASH score were used to evaluate the functional recovery of the affected limb.Results The postoperative follow-up time of the 8 patients was 7 to 23 months,with an average of(14.1±6.1)months.The X-rays were conducted at 3 to 6 months after surgery,and the results showed fracture healing,no further dislocation of the sternoclavicular and acromioclavicular joints,and no surgical related complications.At the last follow-up,all patients had returned to normal life.The average forward flexion angle of the affected shoulder joint was(155.0±19.5)°,and the average abduction angle was(149.0±10.9)°.The average Constant score was 85.5±6.7,the average ASES score was 88.3±5.6,and the average DASH score was 5.6±3.1.Conclusion The bipolar clavicle injury is very rare,often accompanied by concurrent and multiple injuries.The mechanism of injury and the selection of treatment options are still controversial.The appropriate treatment plan can achieve satisfactory clinical efficacy.
