目的 探讨应用微型可吸收骨锚钉固定掌指关节韧带撕脱骨折的手术技术和临床疗效。方法 自2019年1月至2023年6月,我们采用微型可吸收骨锚钉治疗掌指关节韧带撕脱骨折患者15例。采用微型可吸收骨锚钉重建韧带止点,固定撕脱骨折块,术后进行早期适当功能锻炼。结果 术后15例患者均获得随访,时间为6~28个月,平均15。2个月。骨折愈合时间为1~3个月,平均1。5个月。术前、术后患者疼痛视觉模拟评分(visual analogue scale,VAS)分别为7。1±1。4和0。4±0。8,术前、术后差异有统计学意义(P<0。05)。末次随访手指关节总主动活动度(total active motion,TAM)评分:拇指优4例,良1例。其余手指优9例,良1例。结论 微型可吸收骨锚钉治疗掌指关节韧带撕脱骨折,可有效重建韧带止点,固定牢固,患者术后关节功能恢复满意,疗效可靠。
Application of microabsorbable bone anchors for the fixation of metacarpophalangeal ligament avulsion fractures
Objective To investigate the surgical technique and clinical efficacy of applying microabsorbable bone anchors for the fixation of metacarpophalangeal joint ligament avulsion fractures.Methods From January 2019 to June 2023,15 patients with metacarpophalangeal joint ligament avulsion fractures were treated using microabsorbable bone anchors.Microabsorbable bone anchors were used to reconstruct the ligament insertions and fix the avulsion fracture block,and appropriate functional exercises were performed in the early postoperative period.Results After surgery,all 15 patients were follow-up for a period of 6 to 28 months with an average of 15.2 months.The healing time of the fracture was 1 to 3 months,with an average of 1.5 months.The visual analogue scale(VAS)for preoperative and postoperative patient pain were 7.1±1.4 and 0.4±0.8,respectively,with statistically significant differences(P<0.05).At the last follow-up,the total active motion(TAM)score of the finger joint was excellent in 4 cases and good in 1 case for thumb,while excellent in 9 cases and good in 1 case for other fingers.Conclusion The treatment of metacarpophalangeal joint ligament avulsion fracture with microabsorbable bone anchors can effectively reconstruct the ligament insertions and fix firmly.The postoperative joint function recovery of patients is satisfactory,and the therapeutic effect is reliable.
