Clinical efficacy of single plane oblique osteotomy through the base of the first metacarpal bone in the treatment of old intra-articular fractures of the first carpometacarpal joint
Objective To investigate the clinical efficacy of single plane oblique osteotomy through the base of the first metacarpal bone for the treatment of old intra-articular fractures of the first carpometacarpal joint.Methods From January 2019 to November 2023,26 patients with old intra-articular fractures of the first carpometacarpal joint were treated with single plane oblique osteotomy through the base of the first metacarpal bone.The fractured end of the articular surface was exposed by downward flipping of the osteotomy block,and the uneven articular surface fracture was corrected after cleaning the joint cavity.The fracture block was then reduced and fixed with Kirschner wires.Results All patients were follow-up for 6 to 24 months with an average of 15 months.The fractures all achieved bone healing,with a healing time of 2 to 4 months with an average of 3 months.There were no complications such as joint stiffness or pain.According to the upper limb functional evaluation criteria of the Chinese Society of Hand Surgery,the thumb function was evaluated as excellent in 15 cases,good in 8 cases,and poor in 3 cases,with an excellent and good rate of 88.46%.Conclusion Single plane oblique osteotomy through the radial side of the base of the first metacarpal bone can obtain good curative effect in the treatment of old intra-articular fractures of the first carpometacarpal joint.
