Clinical observation of platelet-rich plasma combined with peripheral nerve release in the treatment of diabetic foot ulcer wounds
Objective To explore the clinical efficacy of local injection of platelet-rich plasma(PRP)combined with peripheral nerve release in the treatment of diabetic foot ulcer wounds.Methods A retrospective study was carried out on 162 patients with diabetic foot who were admitted to our hospital from January 2020 to June 2023.Among them,43 cases in the control group(Group A):control of infection,routine treatment of wounds,including surgical debridement of wounds,red and blue light treatment,etc;38 cases in the nerve release group(Group B):the conventional wound treatment method was the same as the control group,and peripheral nerve release surgery was performed on this basis for decompression treatment;46 cases in the PRP group(Group C):the conventional wound treatment method was the same as the control group,and the wound was regularly injected with PRP locally;35 cases in the combination treatment group(Group D):the conventional wound treatment method was the same as the control group,and peripheral nerve decompression treatment was performed on this basis.At the same time,PRP local injection was regularly used on the wound.The wound healing rate,healing efficacy,and degree of limb pain among four groups were compared.Results After 3 weeks of treatment,the wound healing rate of Group D was(68.39±5.64)%,which was higher than that of Group A(52.21±4.83)%,Group B(57.94±5.12)%,and Group C(62.91±5.38)%,and the difference was statistically significant(P<0.05).After 6 weeks of treatment,the effective rate of wound treatment in Group D was 91.43%,which was higher than Group A(74.42%),Group B(81.58%),and Group C(84.78%).The difference between Group D and Group A was statistically significant(P<0.05),but there was no statistically significant difference between Group D and Group B and Group C(P>0.05).The visual analogue scale(VAS)for pain in Group D was(1.34±0.37)points,which was lower than Group A's(2.48±0.55)points,Group B's(1.74±0.46)points,and Group C's(1.85±0.49)points,and the difference was statistically significant(P<0.05).Conclusion The PRP local injection combined with peripheral nerve release can effectively control the infection of diabetic foot ulcer wound,effectively relieve the pain of affected limbs,accelerate the healing speed of diabetic foot wound,shorten the wound healing time,and improve the wound healing rate.
