摘要
目的 探讨全镜下Broström术结合线带加固修复距腓前韧带(anterior talofibular ligament,ATFL)治疗慢性踝关节外侧不稳定的临床疗效.方法 本研究选取2022年1月至2022年12月在无锡市第九人民医院治疗的38例慢性踝关节不稳患者,均采用全关节镜下Broström术结合线带加固治疗.术后采用早期康复方案,采用视觉模拟评分法(visual analogue scale,VAS)、踝-后足功能评分(American Orthopedic Foot and Ankle Society,AOFAS)评价功能,并记录韧带愈合及并发症的发生情况.结果 随访时间为(14～24)个月,平均(19.1±3.4)个月,无并发症发生,术后3个月MRI提示韧带愈合,治疗3月、6月和术后12月后VAS评分及AOFAS评分均优于治疗前(P＜0.05),且随着随访时间延长,VAS评分及AOFAS评分进一步改善.结论 采用全关节镜下Broström术结合线带加固治疗慢性踝关节不稳可有效重建踝关节稳定性,可早期恢复运动能力,临床疗效确切.
Abstract
Objective:To explore the effectiveness of all-arthroscopic Brostrom technique combined with suture tape reinforcement to repair Anterior talofibular ligament(ATFL)in the treatment of chronic lateral ankle instability.Methods:38 patients with chronic ankle instability who were treated in Wuxi Ninth People's Hospital from January 2022 to December 2022 were retrospectively analyzed.All patients were treated with all-arthroscopic Brostrom combined with suture tape reinforcement.Weight-bearing training was carried out early after operation.The visual analogue scale(VAS)and American Orthopedic Foot and Ankle Society(AOFAS)scores before and after treatment were used to evaluate the comprehensive effectiveness.The ligament healing and complications were recorded.Results:The mean follow-up time was(19.1±3.4)months,with 14～24 months.No complications occurred,and MRI showed that the ligament healed at 3 months after operation.The VAS scores and AOFAS scores at 3,6,and 12 months after operation were better than those before treatment(P＜0.05),and the VAS scores and AOFAS scores were further improved with the extension of follow-up time.Conclusion:Arthroscopic Brostrom technique combined with suture tape reinforcement for the treatment of chronic ankle instability can effectively reconstruct the stability of the ankle joint,help the patients recover early movement ability,and has a definite clinical effect.
