基于免疫-炎症轴的化斑汤合参苓白术散预防放疗放射性肺损伤的临床效果

目的 探讨化斑汤合参苓白术散对癌症放疗放射性肺损伤(RILI)的预防效果及对免疫-炎症轴的影响.方法 将2021 年1 月—2023 年1 月收治的120 例癌症放疗患者均分研究组和对照组.2 组均持续放疗6 周,放疗期间研究组予化斑汤合参苓白术散治疗,对照组予中药安慰剂治疗.比较2 组放疗效果、RILI发生率,以及放疗前后血清炎性因子[白细胞介素-6(IL-6)、转化生长因子-β(TGF-β)、肿瘤坏死因子-α(TNF-α)],免疫指标(CD3 +、CD4 +、CD4 +/CD8 +)水平,肺功能指标[肺总量(TLC)、用力肺活量(FVC)],血浆外泌体miR-17-5p、miR-7-5p水平.结果 研究组放疗总有效率[91.67%(55/60)]高于对照组[78.33%(47/60)],RILI发生率低于对照组(P<0.05);研究组放疗3 周、6 周后血清TNF-α、TGF-β、IL-6 及血浆外泌体miR-17-5p、miR-7-5p水平低于对照组,CD3 +、CD4 +、CD4 +/CD8 +及TLC、FVC水平高于对照组(P<0.05).结论 化斑汤合参苓白术散对癌症放疗RILI有良好的预防效果,并可减轻炎症反应,改善免疫功能、肺功能,降低RILI发生率,其作用机制可能与抑制miR-17-5p、miR-7-5p表达有关.
Clinical Effect of Huaban Decoction and Shenling Baizhu Powder on Pre-venting Radiation-Induced Lung Injury Based on Immune-inflammation Axis
Objective To investigate the preventive effect of Huaban decoction and ShenlingBaizhu powder on cancer radiation-induced lung injury(RILI)and its effect on immune-inflammation axis.Methods A total of 120 patients undergo-ing cancer radiotherapy admitted from January 2021 to January 2023 were divided into research group and control group.Both groups were treated with radiotherapy for 6 consecutive weeks.During radiotherapy,the research group was treated with Hua-bang decoction and Shenling Baizhu powder,while the control group was treated with placebo.The radiotherapy efficacy,inci-dence of RILI,serum inflammatory factors[interleukin-6(IL-6),transforming growth factor-β(TGF-β),tumor necrosis fac-tor-α(TNF-α)]and immune indexes(CD3 +,CD4 +,CD4 +/CD8 +),pulmonary function indicators[total lung capacity(TLC),forced vital capacity(FVC)],and plasma exosome miR-17-5p,miR-7-5p levels before and after radiotherapy were compared between the two groups.Results The total effective rate of radiotherapy in the research group[91.67%(55/60)]was higher than that in the control group[78.33%(47/60)],and the incidence of RILI was lower than that in the control group(P<0.05).The levels of serum TNF-α,TGF-β,IL-6 and plasma exosome miR-17-5p and miR-7-5p in the research group were lower than those in the control group at 3 and 6 weeks after radiotherapy,while the levels of CD3 +,CD4 +,CD4 +/CD8 +,TLC and FVC were higher than those in the control group(P<0.05).Conclusion Huaban decoction and Shenling Baizhu powder have good preventive effect on RILI during cancer radiotherapy,which can reduce inflammation,im-prove immune function and pulmonary function,and reduce the incidence of RILI.The mechanism of action may be related to inhibiting the expression of miR-17-5p and miR-7-5p.
