急性脑梗死伴肺部感染患者病原菌分布特点及肺部感染对Bax、FEIR、GSH-Px水平影响

目的 研究急性脑梗死(ACI)伴肺部感染患者病原菌分布特点及肺部感染对B细胞淋巴瘤/白血病-2基因关联X蛋白(Bax)、免疫黏附抑制因子(FEIR)、谷胱甘肽过氧化物酶(GSH-Px)水平的影响.方法 选取2020 年5 月—2022 年5 月治疗的ACI合并肺部感染 53 例作为研究组,同期治疗的ACI未合并肺部感染 51 例作为对照组.分析研究组感染病原菌的分布情况,比较2 组血清Bax、FEIR、GSH-Px水平.结果 研究组53 例中真菌感染5 例,占比9.43%,以白色假丝酵母菌为主;革兰阳性菌感染 24 例,占比 45.28%,以金黄色葡萄球菌为主,其次为肺炎链球菌;革兰阴性菌感染24 例,占比45.28%,以鲍曼不动杆菌为主,其次为阴沟肠杆菌、产酸克雷伯菌.研究组血清Bax、FEIR水平高于对照组,而GSH-Px水平低于对照组(P<0.01).结论 ACI伴肺部感染患者病原菌以革兰阳性菌和革兰阴性菌为主,ACI伴肺部感染患者的氧化应激水平较单独ACI明显增加.
Distribution Characteristics of Pathogens in Patients with Acute Cerebral Infarction Accompanied by Pulmonary Infection and Their Effects on Bax,FEIR,and GSH-Px Levels
Objective To investigate the distribution characteristics of pathogenic bacteria in patients with acute cere-bral infarction(ACI)and the effect of pulmonary infection on the levels of B-cell lymphoma/leukemia-2 gene associated X protein(Bax),immunosuppressive factor(FEIR)and glutathione peroxidase(GSH-Px).Methods Fifty-three ACI patients with pulmonary infection treated from May 2020 to May 2022 were selected as the research group,and 51 ACI patients without pulmonary infection treated during the same period were selected as the control group.The distribution of pathogenic bacteria in the research group was analyzed,and serum Bax,FEIR and GSH-Px levels were compared between the two groups.Results In the research group,5(9.43%)of the 53 patients were infected with fungi,mainly Candida albicans.There were 24 ca-ses of gram-positive bacteria infection,accounting for45.28%,mainly caused by Staphylococcus aureus,followed by Strepto-coccus pneumoniae.There were 24 cases of gram-negative bacteria infection,accounting for 45.28%,mainly caused by Acine-tobacter baumannii,followed by Escherichia coli and Klebsiella pneumoniae.Serum Bax and FEIR levels in the research group were higher than those in the control group,while GSH-Px levels were lower than those in the control group(P<0.01).Conclusion The pathogenic bacteria of ACI patients with lung infection were mainly gram-positive and gram-negative bacteria,and the oxidative stress level of ACI patients with lung infection was significantly increased,as compared with that of ACI alone.
