Relationship between Mean Platelet Volume/Lymphocyte Ratio and Endo-scopic Score and Disease Activity in Patients with Crohn's Disease
Objective To explore the relationship between mean platelet volume(MPV)/lymphocyte ratio(MPVLR)and endoscopic score and disease activity in patients with Crohn's disease(CD).Methods A total of 126 pa-tients with CD admitted from October 2020 to October 2022 were retrospectively selected as the research subjects.According to the simplified Crohn's Disease activity Index at admission,the patients were divided into the active group(n =49)and the re-mission group(n =77),and the patients were classified by the Chinese Grading System for Crohn's Disease.Another 100 healthy volunteers who underwent physical examination during the same period were selected as the control group.The clinical data and MPVLR at admission were analyzed.Multivariate Logistic regression was used to analyze the influencing factors of disease activity in patients with CD.The area under the receiver operating characteristic(ROC)curve(AUC)was used to analyze the predictive value of MPVLR for disease activity in CD patients.Results Compared with patients with mild CD based on endoscopic scores,MPVLR levels were higher in severe and moderate patients,and higher in severe patients than in moderate patients(P<0.05).Compared with the control group,the MPVLR level in the active group and the remission group was increased,which was higher in the active group than in the remission group(P<0.05).The AUC of MPV and lymphocyte in predicting disease activity was 0.740 and 0.851,respectively,and the AUC of MPVLR in prediction was 0.915.The proportion of perianal lesions and C reactive protein(CRP)>3 mg/L in active group was higher than that in re-mission group,while hematocrit and mean erythrocyte volume were lower than those in remission group(P<0.01).Perianal lesions,CRP and MPVLR were independent risk factors for disease activity in Crohn's patients(P<0.01).Conclusion High levels of MPVLR can affect the endoscopic grading and disease activity of patients with CD,and the detection of MPVLR may help to determine the active stage of CD and provide a basis for clinical diagnosis and treatment.
