The value of immune-related cytokine detection in auxiliary diagnosis of tuberculous pleurisy
Objective To detect the differential expression of 12 cytokines,including IFN-γ,IFN-α,TNF-α,IL-1β,IL-2,IL-4,IL-5,IL-6,IL-8,IL-10,IL-12p70,and IL-17,in the peripheral blood of patients with tuberculous pleurisy using immunofluorescence assay,and to explore the diagnostic value of these cytokines for tuberculous pleurisy.Methods Suspected tuberculous pleurisy patients were enrolled,clinically diagnosed,and then divided into a tuberculous pleurisy group ( 73 cases ) and a non-tuberculous pleurisy group ( 92 cases ) .The expression levels of 12 cytokines in their peripheral blood were detected using a multiple microsphere flow cytometry immunofluorescence assay.The sensitivity and specificity of these cytokines in diagnosing tuberculous pleurisy were calculated,and the diagnostic value of combined detection of multiple cytokines was analyzed.Results The expression levels of IFN-α,IL-2,IL-6,IL-5,IL-8,IL-1β,IL-10,IFN-γ,IL-4,and TNF-α were significantly higher in the tuberculous pleurisy group compared to the non-tuberculous pleurisy group ( P<0.05 ) .ROC curve analysis revealed that IL-2,IL-5,IL-8,and IFN-γ individually exhibited higher AUC values for diagnosing tuberculous pleurisy.To enhance the diagnostic specificity and sensitivity,a combined analysis of these four cytokines was conducted,which showed that:the combination of IL-2+IL-8+IFN-γ had an AUC value of 0.994,with a sensitivity of 100% and a specificity of 92%;whereas the combination of IL-5+IL-8+IFN-γ had an AUC value of 0.995,with a sensitivity of 98% and a specificity of 94.4%.Conclusion The combined detection of IL-2+IL-8+IFN-γ and IL-5+IL-8+IFN-γ demonstrates high diagnostic value in assisting the clinical diagnosis of tuberculous pleurisy.
