Clinical Features and Influencing Factors of Carbapenem-Resistant Klebsiella pneumoniae Infection
Objective To analyze the clinical characteristics and influencing factors of carbapenem-resistant Klebsiella pneumoniae (CRKP) infection from 2020 to 2023. Methods The clinical data of 288 hospitalized patients with Klebsiella pneumoniae strains identified by bacterial culture were retrospectively analyzed. Patients were divided into a CRKP group (125 cases) and a non-CRKP group (163 cases) according to the drug sensitivity test results. The clinical characteristics and influencing factors of CRKP infection were statistically analyzed. Results The CRKP group had significantly higher APACHE Ⅱ scores,longer hospital stay,longer ICU stay,longer mechanical ventilation time,and more infection sites than the non-CRKP group (t=9.092,4.295,8.006,26.020,11.688;all P<0.05). The proportion of chronic kidney disease (44.00%) and exposure to≥2 types of antibiotics (52.00%) were higher than in the non-CRKP group (25.77% and 20.25%,respectively;x2=10.529,31.778;both P<0.05). Multivariate logistic regression analysis showed that the time of admission to ICU[OR (95%CI):3.034 (1.844~4.992)],the time of mechanical ventilation[OR (95%CI):2.773 (1.788~4.302)],the number of infected sites[OR (95%CI):3.743 (1.980~7.078)],and the type of exposed antibiotics≥2[OR (95%CI):4.104 (1.991~8.459)]were all independent risk factors affecting the CRKP infection of hospitalized patients with Klebsiella pneu-moniae infection (P<0.05). Conclusion CRKP infection is mainly concentrated in the department of Critical Care Medicine and is influenced by many clinical factors. Therefore,targeted prevention and control measures should be taken for high-risk patients in time to reduce the incidence of CRKP infection from the source,so as to curb the further spread of CRKP.
