Relationship Between Autoantibodies SSA,CENPB,and LDH Levels and Interstitial Lung Disease in Patients with Primary Sjogren's Syndrome
Objective To explore the relationship between the levels of autoantibodies against Sjogren's syndrome A (SSA),centromere B (CENPB),and lactate dehydrogenase (LDH) with interstitial lung disease (ILD) in patients with primary Sjogren's syndrome (pSS). Methods This retrospective study was conducted using 74 patients with pSS retrieved from the case system of Haian People's Hospital,treated between June 2022 and June 2024. Patients were divided into the pSS group (n=59) and the pSS-ILD group (n=15) according to the presence or absence of ILD. General clinical data,positive rates of anti-SSA antibodies and anti-CENPB antibodies,and LDH expression were compared. Receiver operating characteristic (ROC) curves analyzed the predictive value of anti-SSA,anti-CENPB antibodies,and LDH levels for pSS-ILD,while multivariate Logistic re-gression identified influencing factors for pSS-ILD. Results The age of onset and disease duration were significantly higher in the pSS-ILD group (54.40±11.31 years,4.09±1.30 years) compared to the pSS group (44.74±9.67 years,2.67±1.11 years,P<0.05).The occurrence of Raynaud's phenomenon was more frequent in the pSS-ILD group (60.00%) compared to the pSS group (28.81%,P<0.05). Positive rates of anti-SSA (86.66% vs 35.59%) and anti-CENPB antibodies (80.00% vs 45.76%) were higher in the pSS-ILD group (P<0.05),as was LDH level (185.26±23.08 vs 160.18±25.99 U/L,P<0.05). Anti-SSA,CENPB antibodies,and LDH levels predicted pSS-ILD occur-rence with respective AUCs of 0.755,0.671,and 0.766.Combined prediction improved performance (AUC=0.918,sensitivity=86.7%,specificity=86.4%). Positive anti-SSA and anti-CENPB antibodies,elevated LDH,onset age>50 years,disease duration>3 years,and Raynaud's phenomenon were risk factors for pSS-ILD. Conclusion Anti-SSA and anti-CENPB antibodies and LDH levels are associated with pSS-ILD occurrence. Combined assessment enhances predictive efficacy. Positive anti-SSA and anti-CENPB antibodies,el-evated LDH,onset age>50 years,disease duration>3 years,and Raynaud's phenomenon are significant risk factors for pSS-ILD.
