二维斑点追踪成像技术评价尿毒症患者血液透析前后左心房功能的应用价值

目的 本研究利用二维斑点追踪成像技术(2D-STE)观测尿毒症患者在血液透析前后,左心房的结构和功能变化.方法 选取2022 年7 月至2024 年6 月江苏省南京医科大学附属明基医院肾脏内科规律性透析的尿毒症患者100 例为观察组,另选取同期健康体检者100 例为对照组.观察组左心房射血分数正常,在血液透析前和血液透析后2h内进行预定检查项目;对照组仅在体检时检测一次,采集参数同观察组.进行常规超声心动图和二维斑点追踪超声心动图(2D-STE),检测LASr、LAScd、LASct,进行统计学分析.结果 与对照组比较,观察组LASr、LASct均低于对照组,LAScd高于对照组.差异均有统计学意义(P<0.05).结论 2D-STE可检出尿毒症患者左心房功能障碍,评估血液透析患者心房功能改善的效果,血液透析有利于左心房功能的恢复.
The Application of 2-Dimensional Speckle Tracking Imaging in Evaluating Left Atrial Function Before and after Hemodialysis in Uremia
Objective The aim is to detect the structural and functional changes of the left atrium in uremia before and after hemodialysis by using two-dimensional speckle tracking echocardiography.Methods 100 cases of uremia at depart-ment of nephrology,Benq hospital affiliated to Nanjing medical university from July 2022 to June 2024 were enrolled as the ob-servation group.The pre-scheduled examination items were executed before and after 2 hours of hemodialysis treatment.100 normal cases whose age and gender distribution matches the observation group were set as the control group.The control group was tested once during the physical examination,and the parameters were same as observation group.The routine echocardio-graphy and 2D-STE were performed to measure LASr,LAScd and LASct,then statistical analysis.Results Compared with the control group,the observation group had lower LASr and LASct,and higher LAScd.The differences were statistically sig-nificant.(P<0.05).Conclusion 2D-STE was capable for measuring left atrial dysfunction in uremia and evaluate the im-provement of atrial function in hemodialysis patients.Hemodialysis is beneficial for the recovery of left atrial function.
