Analysis on the Changes of Serum miR-802 and miR-15a Levels in the Third Trimester of Pregnancy with Gestational Diabetes Mellitus and Their Interaction on Pregnancy Outcomes
Objective To explore the levels of serum microRNA-802(miR-802)and microRNA-15a(miR-15a)in the third trimester of pregnancy in patients with gestational diabetes mellitus(GDM),and analyze their predictive value for pregnancy outcomes and their interaction.Methods A total of 196 patients with GDM in the third trimester of pregnancy who were diagnosed and treated in Tangshan Maternal and Child Health Hospital in Hebei Province from March 2020 to March 2022 were selected as the study group.Meanwhile,196 normal pregnant women of the same period,the same age group and the same gestational weeks were selected as the control group.The general information,blood glucose indexes[fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),2-hour postprandial blood glucose(2 hPG)],blood lipid indexes[tri-glyceride(TG),total cholesterol(TC)],insulin indexes[homeostasis model assessment of insulin resistance index(HOMA-IR),fasting insulin(FINS)]and the levels of serum miR-802 and miR-15a of the two groups were compared.The correlations between the levels of serum miR-802 and miR-15a in the study group and blood glucose indexes,blood lipid inde-xes and insulin indexes were analyzed.The patients were followed up until the end of pregnancy.The levels of serum miR-802 and miR-15a in the third trimester of pregnancy in patients with different pregnancy outcomes in the study group were com-pared,and their associations with adverse pregnancy outcomes,predictive value for adverse pregnancy outcomes and their in-teraction were analyzed.The area under the receiver operating characteristic curve(ROC)(AUC),net reclassification im-provement(NRI)and integrated discrimination improvement(IDI)were used to evaluate the value of combined detection of serum miR-802 and miR-15a in predicting adverse pregnancy outcomes.Results The levels of FPG,HbA1c,2 hPG,TG,HOMA-IR,FINS and serum miR-802 and miR-15a in the study group were higher than those in the control group(P<0.05).The levels of serum miR-802 and miR-15a in the study group were positively correlated with FPG,2hPG,HOMA-IR and FINS(P<0.05).The levels of serum miR-802 and miR-15a in the third trimester of pregnancy in patients with adverse pregnancy outcomes were higher than those in patients with good pregnancy outcomes(P<0.05).High levels of serum miR-802 and miR-15a in the third trimester of pregnancy had a positive interaction in adverse pregnancy outcomes(P<0.05).The AUC of combined prediction of adverse pregnancy outcomes by serum miR-802 and miR-15a was larger than that predicted by a single index,and the combined prediction effect was significantly improved compared with that predicted by a single index(P<0.05).Conclusion The levels of serum miR-802 and miR-15a in patients with GDM in the third trimester of pregnan-cy are increased,and they have a multiplicative interaction on adverse pregnancy outcomes.The combined detection of their levels has a certain predictive value for adverse pregnancy outcomes.
