Analysis of the Relationship between Serum miR-15a-5p,miR-199b-5p,miR-765 Expression and Prognosis in Patients with Coronary Heart Disease Complicated by Pulmonary Infection
Objective To investigate the changes in serum levels of microribonucleic acid(miR-15a-5p),miR-199b-5p,and miR-765 in patients with coronary heart disease complicated by pulmonary infection,and further analyze the re-lationship between the three and prognosis.Methods A total of 296 patients with coronary heart disease admitted to our hos-pital from May 2021 to June 2024 were gathered and assigned into an infected group(n=160)and an uninfected group(n=136)based on whether they developed pulmonary infection.Among them,infected patients were grouped into a good prognosis group(n=112)and a poor prognosis group(n=48)based on their prognosis.QRT-PCR was applied to detect the levels of serum miR-15a-5p,miR-199b-5p,and miR-765.General clinical data were collected and analyzed.Multivariate logistic re-gression was applied to analyze the factors influencing prognosis.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of serum miR-15a-5p,miR-199b-5p,and miR-765 for prognosis.Pearson method was applied to analyze the correlation between serum miR-15a,miR-199b,miR-765 with CRP,PCT.Results Compared with the unin-fected group,the relative expression level of miR-199b-5p in the infected group was greatly lower(P<0.05),while the rela-tive expression levels of miR-15a-5p and miR-765 were prominently higher(P<0.05).Compared with the good prognosis group,the relative expression level of miR-199b-5p in the poor prognosis group was prominently lower(P<0.05),the rela-tive expression levels of miR-15a-5p and amiR-765 were greatly higher(P<0.05),and the serum levels of CRP,PCT,and BNP in the poor prognosis group were greatly higher than those in the good prognosis group(P<0.05).CRP,PCT,miR-15a-5p,and miR-765 were risk factors affecting prognosis(P<0.05),while miR-199b-5p was a protective factor affecting prognosis(P<0.05).The AUC of miR-15a-5p,miR-199b-5p,miR-765,and their combined prediction for prognosis were 0.838,0.815,0.872,and 0.957,respectively.The AUC of their combined prediction was prominently better than that of miR-15a-5p(Z=3.457,P<0.01),miR-199b-5p(Z=3.995,P<0.01),and miR-765(Z=2.331,P=0.020)alone.MiR-199b-5p was negatively correlated with CRP and PCT(P<0.05),while miR-15a-5p and miR-765 were positively corre-lated with CRP and PCT(P<0.05).Conclusion In patients with coronary heart disease complicated by pulmonary infec-tion,the levels of serum miR-15a-5p and miR-765 are elevated,while the level of miR-199b-5p is reduced,which is correla-ted with the prognosis of patients.
