Preoperative D-NLR Score Variations Across Different Stages of Ovarian Cancer and Its Clinical Significance
Objective To investigate the changes and clinical significance of preoperative D-NLR(D-dimer to Neu-trophil-to-Lymphocyte Ratio)scores in ovarian cancer at different stages.Methods A retrospective study was conducted on 269 ovarian cancer patients who underwent surgical treatment in the gynecology department of the second affiliated hospital of Shaanxi university of chinese medicine June 2012 to June 2023.According to the International Federation of Gynecology and Obstetrics(FIGO)staging criteria,they were divided into an early stage group(107 cases)and a late stage group(162 ca-ses).Additionally,159 ovarian benign tumor patients who underwent surgical treatment during the same period were selected as the benign group..Based on the ROC curve,the cutoff values for D-dimer and NLR were set at 1.45 and 2.46,respec-tively.Subsequently,the D-NLR scores were categorized into three groups:D-NLR score 2,D-NLR score 1,and D-NLR score 0.Nonparametric tests and chi-square tests were used to compare differences in continuous and categorical variables a-mong the groups.Spearman's rank correlation test was employed to assess the correlation between D-NLR scores and various clinical indicators in ovarian cancer patients.Results Statistically significant differences were observed between ovarian cancer patients and those with benign ovarian tumors in terms of age,fibrinogen(FIB)levels,neutrophil counts,D-dimer levels,the number of cases with serum CA125 levels≥35 U/mL,the number of postmenopausal cases,and NLR values(P<0.05).Among patients with ovarian cancer at different stages,statistically significant differences were found in FIB levels,neutrophil counts,lymphocyte counts,platelet counts,D-dimer levels,the number of cases with serum CA125 levels≥35 U/mL,PLR values,NLR values,and the number of ascites cases(P<0.05).D-NLR scores were significantly positively corre-lated with FIGO stage,CA125,and FIB in ovarian cancer patients(P<0.05),but not significantly correlated with age or menopausal status(P>0.05).Conclusion Preoperative D-NLR score,a novel inflammation-based grading system,exhib-its a significant positive correlation with the FIGO stage of ovarian cancer and can serve as an important indicator for assessing ovarian cancer staging.
